Miner was removing bolts from drive belt guard on VSI take-out belt when foot slipped and miner fell into a troughing idler cracking a rib.

Miner was re-setting stop blocks at base of stacker conveyor, when lost balance and mashed finger between conveyor frame and block.

Miner was cleaning spillage off conveyor belt at tail section between guard and tail box, when he stepped off the concrete block supporting the tail section and twisted right ankle inward.

Miner was cleaning stone from grizzlies when a stone slid from top of grizzlies smashing right ring finger between two pieces of stone.

Slid hammer forward to attach to winch line to pull hammer from bed of pickup, hammer slipped and landed on finger. Driller was wearing gloves.

Employee was stepping down off a piece of our water truck, when he slipped and reached to catch his balance. While reaching he cut his index finger on a piece of metal. The cut required six stitches. No lost workdays.
