The employee was operating a skid steer for snow removal. The bucket caught the scale platform and threw employee forward striking the safety bar with employee's chest.

Employee was cutting out a wear plate with a torch when hot metal dropped onto boot. The employee did not notice until it burned through boot

Miner was dismounting service truck and as he turned to dismount backwards lost balance and fell onto other miner. Other miner (miner on ground) ended up with sprained left knee and ankle resulting in restricted duty.

Miner was removing collar from speed reducer on head pulley of agg bin and cut his left index finger on the shaft key way as the collar slid off. Miner received 12 stitches to left index finger.

Miner was exiting mill office coming down steps and twisted ankle on last step. Visit to doctor and x-rays showed that right ankle was broken.

Miner was conducting demolition work removing a wall to create larger storage area in warehouse. While pushing down a wall by hand, a nail punctured his right palm. Miner was taken to medical facility for a tetanus shot, and at that time prescription antibiotics were issued making this injury reportable.

Miner was removing bolt from lid on long miami screw to remove lid, when wrench slipped, causing miners right elbow to strike bolt on other side of lid. Skin on elbow was broken and prescription for antibiotics was issued by doctor.

Miner was punching holes in skirtboard rubber when a shard of metal came off of punch embedding itself into his left hand. This accident is reportable because miner recieved prescription antibiotics to prevent infection.

Fall to the walkway or working surface

Miner was walking from baghouse to showerhouse and slipped on ice spraining right shoulder. At time of doctors visit, was issued prescription pain meds.

Miner was adjusting gate on air slide in mill and mashed ring finger of right hand. No stitches, no cut, no broken bones - DOCTOR PRESCRIBED PRESCRIPTION STRENGTH IBUPROFEN (800 mg.) FOR PAIN

Miner was burning holes under crusher for wear plate installation, when a piece of slag entered his right ear, causing a minor burn. This resulted in a trip to Urgent Care, at which point prescription antibiotics were prescibed.

Miner was walking from the stone house to the shower room to go home and slipped on a frozen rut made from a loader tire and twisted his left knee.

Miner was removing bearing from housing with hammer and punch when a piece of the outer race chipped off and struck him in the left leg; continued working and another piece chipped off and struck him in the right leg.

Miner was removing exhaust system from water truck when a peice of rust broke free and got in his eye.

While replacing v-belts on crusher drive, left hand fingers were caught between v-belt and sheave, smashing middle left finger and seperating fingernail.

Lowering tools by rope to maintance truck below. Retrieving rope to lower welding leads, rope hung up, employee jerked rope to free it, rope came free allowing employees hand to jerk up and cut right little finger outside by knuckle. Right hand struck metal siding. Received 4 stiches in emergency room. Return to work 11/15/06.

Employee was assisting maintenance in pulling a replacement conveyor onto the conveyor frame. A 1/2 inch manila rope was being used to pull the conveyor belt. The belt hung up causing the rope to break. The employee at that time had the rope under his left arm and was pulling. The broken rope pulled loose causing rope burns on right wrist and under left arm pit.

Fall from machine

Employee was assisting maintenance personnel changing a hydraulic apron cyclinder on our 1615 Hazemag. He was standing on the belt guard on the left side of the crusher, his foot slipped causing him to twist his right knee which caused him to lose his balance and fall about two foot to the floor. Employee was taken to the local hospital to have his right knee x-rayed.

EE WAS COMMING OUT OF MILL BUILDING DOWN FLIGHT OF 3 STEPS, ON BOTTOM STEP EE TWISTED HIS LEFT ANKLE. HE CONTINUED TO WORK THE BALANCE OF THE SHIFT AND THE FOLLOWING DAY ON 9-11-030 WHEN HE CAME TO WORK THE ANKLE WAS SWELLED UP AND HE WASTAKEN TO DR.

EMPLOYEE WAS RETURNING FROM CHANGE ROOM TO HIS WORK LOCATION, ACCENDING STEPS TO GO INTO BUILDING HE SLIPPED ON ICE AND FELL ON STEEL STEPS.

INSTALLING STEP BRACKET ON REAR OF 980 F CAT LOADER. BRACKET HAD THREADED HOLES, PUT BOLT IN BRACKET AND TIGHTENED WITH AIR WRENCH, THE BRACKET ROTATED AROUND AND STRUCK EE ON LEFT THUMB.

DID NOT COMPLAIN OTHER THAN SAI D HE WAS SORE. 11-28-00 CAME TO WORK AND COULD HARDLY MOVE RIGHT ARM. TOOK TO ER LOCAL HOSPITAL. RECEIVED SHOT AND SLING. TO BE OFF UNTIL 11-30-00. DR. NOW THINKS POSSIBLE ROTATOR CUFF PROBLE
