Miner walked down the walkway ramp to the boat dock and stepped from ramp to dock (an 8" tall riser ht.)Felt back pain. Did not slip or fall . We took him to E.R. Diagnosis was muscle spasms or sprain. Adm. and injection, 2 Rx's , told to stay home today and apply ice. Can repot to work on 7/21/11 if feeling ok.

Showing all 11Removed 1 section of dredge pipe (mounted on a pontoon) and pushed it to the bank of the lake with motorized work boat. The pipe ran into the bank causing pipe to slip over edge of work boat and pin miner's left leg between the pipe and bench seat of the work boat.
Walking near the booster pump and stepped into a depression or uneven area of the ground. EE went to a chiropractor on 10/31/08.
Miner was tightening packing gland on cyclone pump. Wrench slipped oof of nut, hand struck a hose clamp. Miner was wearing gloves, however, the clamp cut through glove and lacerated right thumb. Required 3 stitches, tetanus vaccination and Rx anitiodic.
2 men working on a dredge sleeve, in process of bolting up the sleeve to pipeline the dredge sleeve fell and EE grabbed one end of it to keep it from going into the lake. The suddent jolt caused an acute strain of his left shoulder
Miner was removing pump packing from a booster pump, when he lost balance & caught himself before falling and then felt something wrench back. He later felt back pain and will be going to a chiropractic clinic for treatment, as soon as he can get an appointment.
UNLOADING JUNK IRON AND A PIECE FELL ON EE'S FOOT. NO BROKEN BONES, BUT FOOT WAS BRUISED. PHYSICIAN INSTRUCTED EE TO STAY OFF FOOT 2 DAYS THEN REPORT BACK TO WORK.
Affected miner & two other miners were lifting a cable block from the rear of a mobile crane. Before the block was completely lowered to the ground, the other two miners let go of the block, which caused the affected miner to feel a jolt. After feeling discomfort for 1 week, the affected miner went to a chiropractor and received one treatment on 10-28-05.
MINER WAS TIGHTENING A CLAMP ON A DREDGE SLEEVE. THE CLAMP BROKE, CAUSING MINER TO LOOSE BALANCE AND FALL AGAINST A BRACE, RESULTING IN MINER'S BUTT STRIKING AGAINST THE BRACE, RESULTING IN FR ACTURED TAILBONE.
EE WAS HELPING REMOVE A COLLECTOR CHUTE FROM THE SCREENING PLANT AND WHILE IT WAS BEING LOWERED IT SWANG AND STRUCK THE EE ON THE RIGHT SIDE OF HIS HEAD. LACERATION REQUIRED 7 SUTURES.
