EE was holding temporary metal plate while another EE was using a 4 inch grinder with cutoff wheel to remove tack welds. The grinder kicked out and cut injured EE's finger (right pinky).

Employee was involved in NWEM Training and being tasked training an operation of a haul truck. EE was riding along with another employee. They stopped and pited the truck. When employee was climbing down from the the haul truck EE reports that EE missed the last step and rolled EE's ankle.

Employee was getting out of the truck on a stripping shot and stepped down from the truck. Where a dip in the ground was that EE didn`t notice and rolled EE's left ankle. When EE fell down EE heard and felt a pop in EE's ankle as EE was falling down EE also jammed EE's left pinky when EE tried to catch self.

Employee reported they were swinging a large sledge hammer while installing the gabion plate back in the grizzly. In order to do that they had to swing a large (heavy) sledge hammer to beat rocks out. *Employee did not report injury until 11/29/2021*
IE was removing a tension bolt on a screen to change screen. Utilizing an impact wrench to remove the bolt ee was guiding the washer/bolt with left hand. EE activated the impact wrench and ee fingers were caught/pinched/grabbed and caused ee to lose skin/tissue on palm side of middle finger.

Employee was carrying ANFO bags from the truck to the shot, about 30 feet away from each other. As EE walked up a slight incline to the shot, EE felt a very painful pop in right calf muscle and EE immediately went to the ground. After sitting on the ground for a few minutes, employee got up to see if EE could walk and couldn't bare much pressure on the right leg without pain

EE injured left index finger when it was struck by a hammer. EE was holding a punch tool and another ee swing the hammer, missed the tool and struck the injured ee hand.
Injured employee reports that they have bi-lateral carpal tunnel syndrome caused by 36 years of working at Magruder Limestone performing general labor, equipment operation, and various job activities. IE reports these include turning wrenches, hammering, changing screens, operating equipment, welding, shoveling, picking, jack hammer.

EE reports that they first swung a hammer and struck right hand pinky finger against metal frame of conveyor and then shortly after that EE reports that while using a socket wrench it slipped off the bolt and they struck the same area on their right hand against a belt clamp.

After cleaning cone crusher employee exited by pulling himself up to top. A shoulder sprain occurred with rotator cuff tendonitis. Corrective action will involve fabricating a portable ladder with attachments and non-slip rungs.
