EE was holding temporary metal plate while another EE was using a 4 inch grinder with cutoff wheel to remove tack welds. The grinder kicked out and cut injured EE's finger (right pinky).

Using hammer to remove ball bearing from shaft, a small piece of metal (less than 3mm) went into skin in neck area

Employee was involved in NWEM Training and being tasked training an operation of a haul truck. EE was riding along with another employee. They stopped and pited the truck. When employee was climbing down from the the haul truck EE reports that EE missed the last step and rolled EE's ankle.

Employee is reporting bi-lateral carpal tunnel to wrist/hands from operating a Cat 992D pit loader. Employee is also complaining about shoulders hurting.

Climbing down from conveyor lost footing and fell about 2 feet onto catwalk landing on metal object.

EE reported EE was tightening a hydraulic hose with a wrench. While pushing on the wrench something popped in EE's elbow.

As the employee was climbing out of a bin the employee's foot slipped and employee's arm got caught on an old bolt causing a puncture wound and trauma to the arm.

Employee reported they were swinging a large sledge hammer while installing the gabion plate back in the grizzly. In order to do that they had to swing a large (heavy) sledge hammer to beat rocks out. *Employee did not report injury until 11/29/2021*

While working on a rock drill, employee used the upper arm, backside to push hoses up and away to move a bracket. Shoulder popped.

Employee was carrying ANFO bags from the truck to the shot, about 30 feet away from each other. As EE walked up a slight incline to the shot, EE felt a very painful pop in right calf muscle and EE immediately went to the ground. After sitting on the ground for a few minutes, employee got up to see if EE could walk and couldn't bare much pressure on the right leg without pain

IE reports these include turning wrenches, hammering, changing screens, operating equipment, welding, shoveling, picking, jack hammer.

Fall from ladders

While working from a ladder the ee was using a hammer to open the door at the bottom of a bin. As ee swung the hammer it caught an electric power cord which threw him off balance and he fell off the ladder.

Installing new training idler on the conveyor, the idler slipped down the frame of the conveyor a few inches and struck employee in the elbow.

While climbing the truck ladder to truck cab the employee missed the top rung and fell to the ground. He landed on the ground on his feet but fell backwards and hit his head on the concrete pad he was parked on. He cut his scalp requiring 4 stitches to close wound. He returned to work the same day.

EE was holding chisel while coworker was hitting it with a sledge hammer. Coworker missed and hit EE's finger, resulting in a chipped bone at the end of EE's finger. This accident was reported to me on 03-18-05.

employee was preparing to get off of conveyor after splicing belt and fell about 15 feet to the ground. Employee landed on the side of his leg bruising his knee.

EE WAS DRIVING THE 966 LOADER DOWN THE HAUL ROADAND IT RAN OUT OF FUEL. HE THEN LOST CONTROL DUE TO THE HYDRAULIC STEERING NOT WORKING. THE LOADER RAN INTO THE ROCK IN THE WASTE PILE AND TURNE D OVER ON ITS SIDE. EE WAS WEARING SEAT BELT AND SUSTAINED A FEW CUTS TO HIS SCALP AND BRUISES. HE RECEIVED 31 STITCHES AND SENT HOME.
