Loading a water hose into a loader bucket the injured employee reported their foot got stuck in the mud. EE could not move and the bucket came down on EE's foot (R).

While removing the cutting edge of the loader bucket, the cutting edge dropped to the floor and caused a bolt to pop up and strike EE in mouth
Employee was getting out of the truck on a stripping shot and stepped down from the truck. Where a dip in the ground was that EE didn`t notice and rolled EE's left ankle. When EE fell down EE heard and felt a pop in EE's ankle as EE was falling down EE also jammed EE's left pinky when EE tried to catch self.

IE was removing a tension bolt on a screen to change screen. Utilizing an impact wrench to remove the bolt ee was guiding the washer/bolt with left hand. EE activated the impact wrench and ee fingers were caught/pinched/grabbed and caused ee to lose skin/tissue on palm side of middle finger.

EE injured left index finger when it was struck by a hammer. EE was holding a punch tool and another ee swing the hammer, missed the tool and struck the injured ee hand.
