Showing all 38EE was shoveling at the plant and felt a burning sensation in lower back. EE worked two days before reporting incident, EE also worked till 03-29-2024. EE did showing up for work on 04/01/2024. As of 04/09/2024, the extent of injury is still unknown.
Employee was working on screen #4 repairing a drive motor when a piece of consolidated limestone dust fell down onto employee from conveyor #33 located above screen #4. Conveyor #33 was not running at the time of accident. The piece of consolidated limestone dust struck employee in the back.
Employee was working on conveyor 7. While removing the chain from the conveyor drive system employee's left hand thumb & forefinger were pinched and lacerated by the chain.
Employee was bent over. Right knee was rested on steel hyd fitting when EE got up EE put all EE's weight against knee. Felt a slight pain, 3 hrs later knee began to swell. EE then received medical treatment. EE has bursitis of right knee.
Employee was straightening a rear access step on a Cat 980G front end loader. The employee was using a steel bar to straighten the step. The bar slipped free causing employee to lose balance and place all his weight and momentum on lower right leg.
EE was painting on processing plant and felt his knee pop when raising from a squatting position. Cont's. to work without restriction on 4/9 & 4/10. Became reportable on 4/11/13 when EE went to doctor due to discomfort. Doctor requested that EE not work for 5 days.
While climbing up to engine compartment on a 300 Komatsu trackhoe using handrails, right forearm came in contact with a sharp edge on a mirror bracket cutting inside.
EE slipped and fell while performing maintenance work on 2/14/2012. This became reportable on 4/3/2012 when he went to doctor for the first time and doctor requested physical/occupational therapy after work hours with no work restrictions.
A hammer was being used to install crusher blow bars. A metal splinter struck employee in bridge of nose.
While removing a lifting eye on crusher lower grate with a hammer. The lifting eye came off and hammer handle pinched the tip of left pointer finger.
While assembling a cone crusher, the bowl adjusting cylinder was working back and forth. While trying to hold the cylinder in place the injured employee pinched his finger causing a cut that required stitches.
EE was working on a conveyor belt in a kneeling position, when he stood up to re-position himself, he felt a pop and muscle strain in his left knee.
EE strained his back while attempting to pick up a 20 lb object. He has not missed any work but on 3/30/11 he saw a doctor for continued discomfort from 9/27/2010 incident making it a reportable injury.
EE strained a muscle in his left thumb while lifting a pail of nuts and bolts. This became a reportable injury on 3/23/10 when his doctor requested physical/occ. therapy and temporary restricted use of his left thumb.
EE twisted left knee while crawling from under 50B haul truck while installing a rear u-joint. Swelling of left knee which did not occur for 3 days, EE worked on 8/28/09 normal duties.
EE was fusing pipe together, while in the process, he injuried his knee. **This was not diagnosed until 7/23/08.
While dismounting from the trackhoe, he strained his lower abdomen.
Was removing top boom cylinder pin with 8# sledge hammer. Pin was overhead. Injured left shoulder, no lost time until today, 1/16/08. Having surgery today, 1/16/08.
Employee was cutting a piece of steel that two other workers were supporting. When he broke through the steel it slid, getting away from the other two workers. The steel plate slid down hitting employee's foot, bending it downward, putting strain in his back.
While pushing off on a stockpile with a front end loader, one wheel of the loader went into a dip in the grade causing a jolt straining his lower back.
Employee was installing steel wear plate in the crushing mill. The load shifted and he twisted trying to control the load, injuring his lower back and leg muscle.
EE WAS HELPING REPLACE A SHAFT IN THE CRUSHER. AS THE SHAFT WAS SWUNG INTO PLACE HE GOT HIS HAND PINCHED BETWEEN THE SHAFT AND HAND RAIL.
LOOKING UNDER A 50 B WABCO HAUL TRUCK IN KINCAID STONE SHOP. EE FELT HIS KNEE POP. WHEN HE STOOD UP HE HAD PAIN AND SOME SWELLING. EE CONTINUED TO WORK. HE HAS BEEN TO DR & SOME THERAPY. DR THORPE DECIDED TO DO SURGERY ON OCT 3, 2005. FIRST LOST DAY OF WORK OCT 2, 2005.
REPETITIVE MOTION UP AND DOWN LADDER OF HAUL TRUCK CAUSED STRAIN ON KNEE. (PENDING WORKMAN'S COMP INVESTIGATION)
EE was performing preshift inspection on Komatsu dozer. He had to add antifreeze. After adding fluid he stepped down off dozer arm onto ground with 5 gal. can in hand. He turned ankle. Ground was level & dozer was parked on rock road. There was light rain the night before. EE wears slip on steel toe boots. They did have a coat of lime mud on bottom.
EE DRIVES AN 85 TON DRESSER 325M HAUL TRUCK. WHILE WAITING TO LOAD 200 FT AWAY FROM LOADING FACE, EE CLIMBED DOWN OFF OF TRUCK. FLOOR WAS DRY AND WEATHER WAS SUNNY. EE WENT TO CLIMB BACK INTO TRUCK AND LEFT KNEE HAD SHARP PAINS. TRUCK HAS NINE STEPS
INJURED WAS REPLACING ROLLER FRAME ON PLANT SURGE BELT. INJURED HIS BACK AND LEFT LEG.
OPERATOR WAS HAVING A LOAD OF DIRT TO WASTE DUMPSITE AS HE COMPLETED DUMPING IS LOAD THE GROUND AT THE REAR OF THE TRUCK STRATED TO GIVE WAY.THETRUCK BEGAN TO SUDE BACKWARD AT THAT POINT THE OPERATOR JUMPED FROM THE CAB TO THE GROUND CAUSING INJURY TO HIS LEFT FOOT AND LOWER BACK. TRUCKSLID APPROX. 30 FT AND WAS STOPPED BY A DIRT BANK AND LOWER BACK. TRUCK SLID APPROX. 30 FT. AND
GREASING EQUIPMENT WHEN HE TRIGGER HIGH PRESSURE HOSE FROM CLOSE RANGE STRIKING HIMSELF IN THE EYE WITH A STREAM OF GREASE.
OPERATOR STOPPED ON HAUL RAMP TO REMOVE LARGE ROCKS FROM HIS PATH. HE KICKED A ROCK OFF THE RAMPAND IN DOING SO STRAINED HIS KNEE
EMPLOYEE WAS REPLACING A WEAR PLATE AT THE CRUSHING PLANT. HE LIFTED A PIECE OF STEEL & INJURED HIS LOWER BACK.
EE WAS STEPPING OFF LOADER, SLIPPED AND FELL, HITTING HIS KNEE ON LOADER STEP.
EE WAS CHANGING SCREENS ON A SCREEN DECK. HE PULLED A SCREEN, SLIPPED AND INJURED HIS BACK.
EE RECEIVED FLASH BURN FROM C0-WORKER WELDING INTHE SAME AREA.
EE WAS IN THE PROCESS OF WASHING UNIT #330 WITH A POWER STEAM JENNY WHEN HE SLIPPED. WHILE TRYING TO CATCH HIMSELF THE LINE OF HOT WATER PRESSURE CAME ACROSS HIS RIGHT FOOT.
DISMOUNTING FROM HAUL TRUCK LADDER SLIPPED & HUNG ON WITH HAND & HURT WRIST
EE WAS LIFTING I BEAM OUT OF ROCK HOPPER. HE LOST HIS FOOTING FELL AND BUMPED HIS KNEE ON A ROCK.
