Over-exertion in lifting objects

EE was shoveling at the plant and felt a burning sensation in lower back. EE worked two days before reporting incident, EE also worked till 03-29-2024. EE did showing up for work on 04/01/2024. As of 04/09/2024, the extent of injury is still unknown.

Struck against stationary object

Employee was bent over. Right knee was rested on steel hyd fitting when EE got up EE put all EE's weight against knee. Felt a slight pain, 3 hrs later knee began to swell. EE then received medical treatment. EE has bursitis of right knee.

While climbing up to engine compartment on a 300 Komatsu trackhoe using handrails, right forearm came in contact with a sharp edge on a mirror bracket cutting inside.

A hammer was being used to install crusher blow bars. A metal splinter struck employee in bridge of nose.

While removing a lifting eye on crusher lower grate with a hammer. The lifting eye came off and hammer handle pinched the tip of left pointer finger.

EE twisted left knee while crawling from under 50B haul truck while installing a rear u-joint. Swelling of left knee which did not occur for 3 days, EE worked on 8/28/09 normal duties.

Was removing top boom cylinder pin with 8# sledge hammer. Pin was overhead. Injured left shoulder, no lost time until today, 1/16/08. Having surgery today, 1/16/08.

LOOKING UNDER A 50 B WABCO HAUL TRUCK IN KINCAID STONE SHOP. EE FELT HIS KNEE POP. WHEN HE STOOD UP HE HAD PAIN AND SOME SWELLING. EE CONTINUED TO WORK. HE HAS BEEN TO DR & SOME THERAPY. DR THORPE DECIDED TO DO SURGERY ON OCT 3, 2005. FIRST LOST DAY OF WORK OCT 2, 2005.

EE was performing preshift inspection on Komatsu dozer. He had to add antifreeze. After adding fluid he stepped down off dozer arm onto ground with 5 gal. can in hand. He turned ankle. Ground was level & dozer was parked on rock road. There was light rain the night before. EE wears slip on steel toe boots. They did have a coat of lime mud on bottom.

EE DRIVES AN 85 TON DRESSER 325M HAUL TRUCK. WHILE WAITING TO LOAD 200 FT AWAY FROM LOADING FACE, EE CLIMBED DOWN OFF OF TRUCK. FLOOR WAS DRY AND WEATHER WAS SUNNY. EE WENT TO CLIMB BACK INTO TRUCK AND LEFT KNEE HAD SHARP PAINS. TRUCK HAS NINE STEPS
