The left rear tire on an End Dump blew out while the employee was at the rear of the truck doing an inspection. The noise of the blow out hurt his ears.
Employee was cleaning up around oil station work area, he stepped on a wooden pallet, twisting and breaking his ankle.
High winds blew dust into employee's right eye under his safety glasses.

Based on investigation it is belived that EE climbed up on top of the pan feeder on an extec crusher while it was in operation. This was in violation of company policy & training. EE went through the crusher and was killed.
Employee was cleaning tailpulley with prybar and belt was turned on. Note: Employee quit going to the doctor.
Driver was getting out of truck, foot slipped and was falling and put right hand on ground to catch himself.
