Victim was scaling in level 9 north 8 when a large rock fell from the roof crushing the cab of the scale machine causing fatal injuries. The accident is still under investigation by MSHA and the mine operator. The root cause analysis is not complete at this time.
APPARENTLY EE ENTERED THE BAGHOUSE ROOM COMPARTMENT SOMETIME BETWEEN 8/5 @ 6 OR 7 PM AND 8/6 @ 4 AM. THE DOOR APPARENTLY SHUT AND LATCHED WHEN THE EE WAS IN THE ROOM. THE CONDITIONS IN THE ROO M WERE HOT AND DUSTY.
THE #2 STONE BIN WITH APPROX. 600 TONS OF CRUSHED STONE FAILED ALLOWING THE CONE SECTION TO FALL THROUGH THE PREP LAB BUILDING. THE QUALITY CONTROL MGR WAS IN THE BUILDING AND WAS CRUSHED BY C OLLAPSING STRUCTURE AND MATERIAL.
EE WAS WORKING ON THE SILVER JET WASHER HIGH PRESSURE AFTER MAKING REPAIRS TO THE WASHER. HE HAD TO REPLACE THE HUBBLE TWIST LOCK PLUG AND CABLE. HE IMPROPERLY WIRE AT WASHER CONTROL. THE 490 VOLTS 3 PHASE PLUG WAS WIRE WITH A POWER WIRE COUNNECTED TO THE GROUND WIRE PUTTING APPROX. 277 VOLTS ONTO THE WASHER FRAME AND ITS METAL ATTACHMENTS.
