EE was moving a slab of sandstone into position to be split, when he cut his left hand on a sharp edge of the stone. EE was given a tetanus injection, as well as several stitches to close the laceration.
Employee was moving a large thin piece of sandstone with the stone standing up on edge. A piece approx 1 Sq Ft. broke off the top edge and struck his right wrist causing a laceration which required 6 sitiches to close.

EE was moving a large piece of stone into position to be split. The stone slipped from his grasp and struck his left leg 3 inches below his knee causing a laceration. He received serveral stitches and returned to full work capasity on 7/16/07.
EE was splitting a slab of sandstone using a stone chisel & 4lb hammer when a metal fragment broke off the chisel & implaed in his abdomen. EE rec'd medical treatment for removal of foreign body & 1 suture to close wound. Released to return to work 6/26/07.
Employee was splitting a slab of sandstone using a stone chisel and 4-LB hammer when a metal fragment broke off the chisel head and impaled in his right thigh. He declined medical treatment at the time of the injury. on May 21,2007 he requested medical treatment and did see a physician.

Employee was lifting a 4" thick slab of sandstone off the top of a large block from which it was split off of, when his feet slipped from under him. The 4" slab fell back onto the block crushing his left index finger between the 4" slab and the block.
Employee was lifting large blocks of sandstone (several tons) with the forks of a front end loader. He sustained a back injury when a block broke and fell from the forks causing the loader to be abruptly bounced upward. Employee will be on restricted work until doctor's release.

While splitting sandstone using a 4-lb. Hammer and stone chisel the hammer glanced off the chisel head striking ee's left index finger resulting in a contusion and laceration requiring 4 stitches.
On 8/25/06, EE requested to see a physician due to pain in his lower abdomen. He first noticed the pain on 8/16/06, but did not become concerned til 8/25/06, when it effected his ability to perform his job. Dr diagnosis: Hernia, requiring surgery to repair. EE returned to work on 8/28/06 in restircted capacity until surgery.
EE was palletizing slabs of sandstone by hand when he sustained a sharp pain to his lower back. Physicians diagnosis: Low back strain. EE will be off work for 7 to 10 days.
EE was attempting to move a slab of sandstone by pushing on it's edge. His left side was facing the stone. He had his left elbow tucked into his left rib cage and left hand on the stone pushing when he sustained a sharp pain to his lower left rib cage area. Physician diagnosis: rib separation.
EE was lowering a slab of sandstone to the ground when he pinched his left middle finger between ground and the stone slab. DR. Diagnosis: laceration & contusion left middle finger. EE lost 3 work days.
Employee was splitting a piece of sandstone using a 4 lb.hammer & a stone chisel when a metal fragment broke off the chisel & embedded in his left hand. EE declined medical treatment at the time, on 8/7/06 EE requested medical treatment for the pain in his hand. Dr. removed the fragment and stitched the wound.
EE was using a 36" pry bar to fift a slab of sandstone, when the bar slipped out from under the stone, causing the stone to fall on his right index finger, resulting in a contusion to his right index fingertip.
Employee requested to see a doctor because of back pain. Work exposure is a contributing factor to the aggravation of a previous back injury he sustained on 6/13/06. Employee was released to full duty on 6/28/06. He is again on restricted work activity (light duty) begining 7/21/06.
EE was using a 36" pry bar to lift a slab of sandstone when the bar slipped out from under the stone causing the stone to fall on his right hand resulting in a contusion to his right ring finger. EE will be performing lite duty work until next DR appt on 7/17/06.
Employee was splitting a block of sandstone using a 12 lb. hammer and stone chisel. He was using his left foot to hold his chisel in position on a bench in the block of stone so he could strike the chisel and split the stone. He inadvertantly missed the chisel and struck his left foot.
EE was splitting a slab of sandstone using a 4lb hammer and stone chisel, when he was struck in the left eye by stone particals, which passed under the lenses of his safety glasses. Particals were removed by a physician. EE lost one day or work.

EE was attempting to move a large block of sandstone (6-7 ton) in the bucket of a front end wheel loader, when the block slid off the bucket causing the loader to be jarred abruptly, resulting in a back strain to EE.

EE was extracting a large block of sandstone from an excavation site using the bucket of his excavator when the bucket slipped from the back of the block causing the excavator to abruptly move backward and forward resulting in EE sustaining a back injury.

EE was lifting a block of stone into position to be split when he sustained a sharp pain in his left shoulder. Physician diagnosis: left shoulder injury, no use of left shoulder for 7-10 days.

EE was splitting a piece of sandstone using a 4lb hammer and stone chisel when a piece of metal from the chisel punctured his left hand. Physician removed metal fragment and used 2 stitches to close laceration. EE returned to work at full capacity on 1/19/2006.
Employee was lifting a block of sandstone using a crowbar when the stone broke in half. This caused him to lose his footing and fall to the ground twisting his lower back. Physician's diagnosis: low back strain. Employee will be off work 7 to 10 days. Next appt. 1-26-06.
EMPLOYEE WAS PALLETIZING PIECES OF STONE WHEN HE SUSTAINED A SHARP PAIN TO HIS LOWER ABDOMEN. DR.'S DIAGNOSIS: ABDOMEN WALL STRAIN. EE RETURNED TO WORK ON 1/16/06 IN A LIGHT DUTY WORK ASSIGNMENT. NEXT DR'S APPT. IN ONE WEEK FOR RE-EVALUATION.
EE was palletizing pieces of patio stone when he sustained a sharp pain in his lower back. Physician deignosis: lumbar strain He lost 1 day of work on 12/14/05. He returned to work on light duty on 12-15-05 until next evaluation of 12/7/05.
EE WAS SPLITTING A BLOCK OF STONE WHEN A PIECE OF STONE FRACTURED OFF THE SIDE OF THE BLOCK AND FELL ON HIS LEFT FOOT. PHYSICIAN DIAGNOSIS: FRACTURE OF TALUS BONE. EE WAS AUTHORIZED TO WORK IN A LIGHT DUTY STATUS UNTIL FURTHER NOTICE.
EMPLOYEE PLACED A HAMMER ON TOP OF A BLOCK OF STONE. THE HAMMER HANDLE WAS PROTRUDING OUT OVER THE EDGE OF THE BLOCK. WHILE MOVING ANOTHER PIECE OF STONE INTO POSITION ALONGSIDE THE BLOCK, THE STONE BEING MOVED CAME DOWN ON THE HAMMER HANDLE CAUSING THE HAMMER TO FLIP UPWARD STRIKING HIM ON THE HEAD.
EMPLOYEE SUSTAINED A STRAIN TO HIS LOWER BACK WHILE LIFTING PIECES OF STONE ONTO A PALLET. PHYSICIAN DIAGNOSIS: LOW BACK STRAIN, NO WORK UNTIL COMPLETION OF PHYSICAL THERAPY (4-6 WEEKS).
EE was splitting a block of sandstone with a hammer and chisel when a fragment of metal broke off the chisel head and empbedded in his left hand between his thumb and forefinger. Physician deiagnsosis: no work until re-evalvation on 11-23-05.
EE WAS MOVING A SLAB OF STONE TO BE SPLIT WHEN HE PINCHED HIS LEFT MIDDLE FINGER BETWEEN THE STONE HE WAS MOVING AND AN ADJACENT STATIONARY BLOCK OF STONE. HE SUSTAINED A LACERATION AND FRACTURE TO HIS FINGER. PHYSICIAN INSTRUCTIONS: NO WORK TILL RE-EVALUATION ON 11-11-05.
