The power to the slope hoist went down about 2:25pm due to storm. MSHA was notified about 2:50pm. The electric company had to replace a pole fuse to repair the issue. The power came up at about 6:30pm.

Employee stated he was walking up a stairway and felt pain in lower calf. Visited doctor and discovered a tear in knee area. Determined by Workers Compensation after investigation this was work related on 9/21/2015.

Customer bulk truck EE was preparing to climb on tank trailer to open the lids for loading. He was at the tie-off rack at Carmeuse and had hooked to the self retracting lanyard. As soon as he placed his second foot and full weight onto the step, (This step is 20-1/2 inches from the ground.) it broke and he fell to the ground, landing on his buttocks.

The employee was stocking brick on work plate form. Employee stated while placing brick on platform page stick came dislodged causing the brick to fall striking his left forearm. Causing fracture to left humorous bone, Brick weight was 126lbs.

While relocating piping to another jobsite an employee was walking along side a all terrain forklift when his foot got under the tire. Employee was taken to emergency room and x ray showed fracture in big toe and a laceration between the toes.

Employee was in the process of changing sprays for #4 kiln. After removing spray and placing on catwalk, the spray bar caught on kiln and pinched his foot between the bar and catwalk.

Employee was using hand grinder to grind down weld. After shutting off and preparing to set the grinder on the bench, the wheel became tangled in the string of his hooded sweatshirt, which then wrapped up and contacted his jaw causing a laceration.

Employee had moved vacuum hose out of the walk way. As he turned to walk away he tripped on his own foot and fell forward. He extended his arms to catch himself and when his left hand hit the floor he incurred a fracture to his wrist.

Employee was using a utility knife to cut a bad end off a 4" plastic corrugated pipe. The knife blade slipped out of the pipe striking the right forearm causing a laceration that required stitches.

While rolling on v-belts for the cooler fan, employee caught his left index finger in between the motor sheave and the belt pinching the end of his finger. The injury was to his left pointer finger.

Two employees were in the process of opening a rail car hopper gate. Both employees were using bars to rotate the gate mechanism when the injured employee felt a pop in his elbow.

The alarm system on the slope hoist indicated a brake issue. Hoist was shut down and repairs made. The brake adjustment relay out of adjustment. Hoist put back into operation at 1:42pm.

Injured was checking oil levels in kiln trunion after attending man lift training for most of the day. Injured became dizzy and blacked out causing him to fall and strike his head causing laceration. Injured was taken to the ER and diagnosed with dehydration contributed to by a medication he was taking, and low potassium levels.

EE dropped a long handled scraper into hydrated lime. The hydrated lime splashed into his eye. He was not wearing his safety glasses properly, they were being worn on the tip of his nose.

While loading shells into the kiln gun a piece of one of the slugs ricochet back out of the kiln after it had been shot and struck EE on his right forearm. It scraped a small section of skin off and the area swelled up. EE was taken to doctor for evaluation. No fractures or stitches.

Lime dust entered employee's left eye while removing safety glasses. Employee flushed eye on day of incident removing the irritant. Reported to work on 3/20/14 stating eye still felt different and requested to see doctor. Prescription eye drops were prescribed.

JLB Trucking (contract coal hauler) was on site preparing to dump a load of coal. Driver said he heard a pop and then smoke and flames entered the cab of his truck (#22.) Driver used his portable fire extinguisher but it did not extinguish the fire. The Gary Fire Department was contacted to fully extinguish the fire.

Employee mashed his finger while swinging a sledge hammer. It appears when he was hitting the side of the slurry bin his hand slid down the handle of the hammer. He continued to swing and his left index finger got mashed in between the handle and a section of plate that wraps around the bin. Employee received 5 stitches.

At approximately 10:00am on 12/20/2013, Kiln 4 coal weigh feeder was undergoing a belt cut which required the temporary removal of the coal feed to the mill. During this time, the coal dust inside the mill ignited causing dust to be expelled from the feeder opening and the hood of the kiln.

Injured was attaching hose to the vac truck while standing on the work platform approximately 5 1/2 feet off the ground, when he lost his footing and fell. Employee was wearing fall protection, including harness and lanyard. The equipment prevented the employee from striking the ground, however it caused him to swing back into the truck causing a rib injury.

Maintenance EE's were striking the end of a screw conveyor with an 8lb sledge hammer and a piece of the shaft flew off and struck the injured employee in the forearm causing a laceration and embedding of metal.
