Employee was exiting the CAT dozer and bumped his knee on the equipment, specifically on the top of the blade. The employee did not want medical attention at the time of the accident but came today asking for medical treatment
Miner was stacking rock in the pit and lifted a rock that was too heavy and hurt left shoulder.
Miner was lifting rock to stack on pallet, and rock slipped out of hand on top of right top foot.
Operator dropped rock on employees hand. Employee has smashed fingers unknown at time of filing if any are broke.
Employee was pushing a rock that was hung in the chopper. He then slipped on loose rock and hydraulic oil, and twisted his back.
Employee was training on a Cat 627G, first day, he was fully trained and was attempting his first pull when he was turning and hit a bump. The employee was "whipped" forward and seated hard. He has a compression fracture of his L2 vertebra in his lower back.
EE was working on our rock chopper. He was removing a rock from the rollers. When the operator rolled the rollers ee glove got caught in the rollers and pulled his hand down into the rollers.
Employee was shutting door on the 966 Unloader (cat loader) employee's foot slipped and he fell off the unloader to the ground (about 8 ft) causing laceration to left knee, requiring stitches along with contusions & scrapes to left side of face.
The employee bruised finger when assisting the loading of a rock onto one of the splitters.
Employee was turning over a stone on the chopper and smashed his thumb between the rock and the chopper teeth. Thumb was split open by the rock.
Employee had gone to see a doctor over the weekend claiming an accident where his left index finger was caught between rocks while working on the chopper. His supervisor had not been informed of an accident. Doctor visit showed fracture to left index finger.
Man loading dump truck from stockpile. Did not set parking brake or chock tires. Truck rolled down hill 300 ft and over 10' embankment.
Unknown at this time. EE was terminated on 7/7/08 with no report of injury. After short term disability benefits expired, former alleged that he was hurt on the property. We were first notified of this event on 8/20/08 by his attorney---no injury occurred on the property.
Electrical outlet shorted out igniting wood interior wall. Fire dept. responded, extinguished fire. Gates were locked; no miners present at time of fire. Approx. time fire dept. was on scene 1 hr 10 min.
Frozen tire tracks (6-8 inches deep) and normal winter ground conditions. Driver claims that bumps caused him to leave seat and strike rib on dump lever. Driver claimed to be wearing seat belt. No damage or defect found on truck.
Tresspassed by opening gate to mine site and was riding an ATV when control was lost.
EMPLOYEE WAS CLEANING FLOOR OF 966F CATERPILLAR LOADER. HE FELT SOMETHING STICK UNDER HIS NAIL. EMPLOYEE TRIED TO REMOVE OBJECT AND HE SAID THAT HIS FINGER WAS SORE IN A COUPLE OF DAYS.
EE was helping install the kiln cover back over kiln after repairs was completed. he reached up to help unhang cover and the beam that ee was holding on to fell hitting him on left leg.
Wet and muddy, EE was filling up machine with water. He was climbing on machine. Went to move a 5 gallon jug of water to the next level of catwalk. His hand slipped off of handrail, he fell against machine bruising his ribs and his leg hit the tracks lacerating his leg.
Employee was fueling D-7 Dozer, when he slipped and caught is finger in the basket, causing a fracture.
Ee was using a pry bar to rotated the tail pulley on a conveyor. The pry bar slipped out catching his hand between the bar and the catwalk on the conveyor causing a laceration to little finger.
Employee was driving a loaded pan around a turn and the pan flipped over and his foot got caught under the rollover protection and crushed his foot.
EE WAS INSTALLING THE BOTTOM PAN COVER ON A D7 DOZER WITH A SHOP FLOOR AS EE TRIED TO ROLL THE JACK UNDER THE MACHINE THE PAN CAME OFF THE JACK & A SHARP EDGE ON THE PAN CUT HIS THUMB ON HIS LEFT HAND.
EE WAS IN THE PROCESS OF ENGAGING AN ELECTRICAL DISCONNECT. WHEN HE PULLED ON THE HANDLE IT SLIPPED FROM HIS HAND, STRIKING HIM IN THE MOUTH CAUSING A CHIPPED TOOTH.
EE WAS GETTING OUT OF LOADER, PUT FOOT ON FIRST STEPDOWN WHEN HE STEPPED ON 2ND STEP HIS FOOT SLIPPED AND HE HELD ONTO A RAIL ON LOADER PULLING HIS RIGHT SHOULDER. SPRAINED RIGHT SHOULDER, ELB OW AND WRIST. STEPS WERE WET IT HAD BEEN RAINING.
EE WAS TAKING D-7 TO SHOP TO FUEL & SERVICE. HE HAD TO TRAVEL DOWN A STEEP INCLINE AT BACK OF RAW MATERIAL SHED. EE HAD THE DOZER IN A HIGH GEAR AS HE WENT DOWN STEEP INCLINE, THE BLADE WENT I NTO GROUND STOPPING THE EQUIPMENT. EE'S HEAD HIT THE WINDSHIELD BUSTING THE GLASS & INJURED EE.
WHILE STANDING NEAR TRACKSIDE OF SHOVEL, EE WAS PINNED BETWEEN LOADER TRACK AND FRONT OF STERLING DUMP TRUCK (MS 9) TRUCK WAS PARKED ON GRADE OF 6-8% UNATTENDED, IT ROLLED INTO LOADER, PINNING EMPLOYEE.
EE WAS CLIMBING DOWN FROM THE REAR OF THE 627 PAN, LOST HIS FOOTHING MISSED STEP SLIPPED AND FELL BACKWARD LOWER BACK INJURY.
A CONTRACT EMPLOYEE WAS ACCUSING SUBJECT OF UNSAFE DRIVING TO MINING SUPERVISOR, CLAIMING HE HADSPLASHED MUD ON HIM THE PREVIOUS FRIDAY. SUBJECTWAS WITHIN HEARING DISTANCE & DENIED THIS, CALLI NG HIM A LIAR. AT THIS POINT CONTRACT EMPLOYEE STRUCK VICTIM IN BACK OF THE HEAD WITH HIS FIST.HE CONTINUED TO HIT VICTIM, KNOCKING HIM TO THE GROUND. TWO CO-WORKERS PULLED HIM OFF.
TWO EMPLOYEES WERE CHANGING A RIPPER TOOTH ON A D8 DOZER. INJURED BEAT AROUND THE TOOTH TO LOOSEN IT. HE HIT THE TOP EDGE, A CHIP CAME OFF PENETRATING HIS LEFT LEG.
