Miner was stacking rock in the pit and lifted a rock that was too heavy and hurt left shoulder.

Miner was lifting rock to stack on pallet, and rock slipped out of hand on top of right top foot.

Operator dropped rock on employees hand. Employee has smashed fingers unknown at time of filing if any are broke.

Employee was pushing a rock that was hung in the chopper. He then slipped on loose rock and hydraulic oil, and twisted his back.

EE was working on our rock chopper. He was removing a rock from the rollers. When the operator rolled the rollers ee glove got caught in the rollers and pulled his hand down into the rollers.

The employee bruised finger when assisting the loading of a rock onto one of the splitters.

Employee was turning over a stone on the chopper and smashed his thumb between the rock and the chopper teeth. Thumb was split open by the rock.

Employee had gone to see a doctor over the weekend claiming an accident where his left index finger was caught between rocks while working on the chopper. His supervisor had not been informed of an accident. Doctor visit showed fracture to left index finger.

Unknown at this time. EE was terminated on 7/7/08 with no report of injury. After short term disability benefits expired, former alleged that he was hurt on the property. We were first notified of this event on 8/20/08 by his attorney---no injury occurred on the property.

Over-exertion in lifting objects

EE picked up a rock and started complaining about his testicles and his side. He went to the hospital emergency room and was released the same day.

Struck by falling object

EE walked between two pallets while on his break. A rock fell off one pallet pinning him to the second pallet. He does not remember leaning against the rock that fell.

Fall onto or against objects

Employee picked up a 1"" rock to carry it to the pallet. He stepped back tripping over another rock on the ground hitting the back of his neck. There was no cut, only whiplash.

Employee is diabetic. Blood sugar was low, tried to eat a candy bar, gagged, had a seizure, bit his tongue badly on both sides enough to bleed profusely. Discharged from hospital, but will not be allowed to return to work until we have a statement from a doctor that he is fit to return to work. There was no accident.

EE and other ee were carrying a rock. He was waking backwards to a pallet to place the rock on. He looked towards his right to make sure there were no rocks in his way when he tripped over a rock on his left. He fell backwards and hit his head on a rock causing a laceration.

Employee was operating the chopper when a medium sized rock fell on his hand and bruised his left ring finger.

EE WAS SPLITTING ROCK WITH CJ SPLITTER. EE PUSHED LARGE SLAB OF ROCK UP TO SPLITTER TEETH. HIS HANDS WERE PLACED AT THE FRONT AND REAR OF THE ROCK WHEN PUSHING. EE'S FINGER HIT THE SPLITTER TEETH WHEN PUSHED FORWARD. ACCIDENT CAUSED SMALL CUT TO RIGHT INDEX FINGER, CAUSING STITCHES TO BE ADMINISTERED (NO AMPUTATION).
