Employee was re-spooling a cable winch. Employee was using left hand to guide the steel cable onto the spool. EE's left thumb got too close to the spool and was pinched by the cable and spool, causing a tear/laceration to the left thumb.

Helping install a hold-down roller on a conveyor. Employee was watching co-worker use a hammer and chisel to seat a bearing onto the roller shaft. As the co-worker struck the chisel with the hammer, the employee was struck in the right eye by a flying object (small piece of metal)

The injured miner bent over to pick up a pry bar, turned and tripped on a rock and fell onto left shoulder.

Changing hammers in HSI crusher, employee was using a hammer to drive wedge out . . . struck his own left index finger with the hammer.

He stepped off a ladder and twisted right ankle when foot contacted ground. Worked for rest of day and next. 6/16/08 went to doctor to have it looked at and doctor had him stay home 2 days. Returned to work 6/18/08.

On 3/19/07, we discovered that someone had broken into the quarry, and cut the fill hose on a 20,000 gal. diesel fuel tank. About 4,000 gallons of fuel spilled onto the ground, outside the secondary containment. KY DEP, NRC, MSHA, and local police/fire departments were all notified. TPM Group was contacted to help coordinate the cleanup efforts.

Lifting rocks out of jaw crusher by hand after a discharge belt plugged it. Pulled muscle in back.

Employee was exiting the cab of the drill. He stepped down on the 1st step, then missed the 2nd step & fell about 4', landing on the ground. He stated that he was unable to grab the handle in time to keep him from falling. Other than failure to maintain 3-point contact, there appear to be no contributing factors causing the incident (moisture, wind, defective equipment, etc.).

Technician was installing cover on air conditioning fan when his hand slipped and his finger hit the fan. Did not receive stiches, but had skin graft later.

Our T371A CAT haul truck was hualing rock from underground mine to a stockpile located at the surface. While backing up to the stockpile the haultrucks brake chamber failed causing the truck to roll back a slight grade into the stockpile. The driver complianed of back pain, but finished out his shift.

Accident occurred while removing brush from beaver dam at quarry lake drain, slip and fall causing back pain the next day.

LOADER BUCKET EDGE STRUCK CORNER OF PILLOW H 20 WEST TURNING LOADER SHARPLEY TO RIGHT, TWISTING OPERATOR IN SEAT.

EE WAS PUTTING WATER ON HEADER 15 EAST WHEN A PIECE OF ROCK FELL ABOUT 10 FOOT STRIKCING HIM ON SIDE OF HARD HAT AND STEEL TOE BOOT. THE AREA WAS INSPECTED AND NOTHING WAS FOUND EARLY IN THE S HIFT.

EE WAS ATTEMPTING TO BACK A 769 CAT 35 TON TRUCK UNDER A RAISED LOADER BUT FAILED TO STOP BEFORE STRIKING A LIME STONE PILLER, NO DAMAGE WAS DONE TO TRUCK.

AIRING UP TIRE ON SERVICE TRUCK (LOOKED A LITTLE LOW) WHEN SPLIT RIM BLEW APART STRIKING GROUNDMAN ON RIGHT LEG.

EE ALLEGES THAT WHILE BACKING TRUCK UNDERGROUND, BRAKE SYSTEM FAILED CAUSING PAIN TO NECK AND SHOULDER. WE CONTEST THIS ACCIDENT!!

EE WAS CLEANING MUD FROM CONVEYOR BELT IDLER WHEN A LOOSE SLEEVE GOT CAUGHT ON BELT PULLING HIS ARM AGAINST MOVING CONVEYOR, BADLY TEARING AT HIS ARM.
