While setting the feeder, EE was putting a crib block under feeder when one of the blocks crushed out letting the feeder set down on EE's hand.
Employee was loading structure on 001 section. Scoop pulled forward catching waterline, bowing waterline over which pushed structure over. Structure caught employee's leg causing injury to left knee.
Employee missed step on ladder on way down and injured right calf muscle.

Found roof fall in the no. 1 entry approx. 400 ft from the face. Fall occurred in the intersection. 20 ft wide, 20 ft long, 6 ft thick. This is active non-producing status. No coal being produced at this mine. No one was in the mine when the fall occurred.
Somewhat of a fault area, sandstone roof inby fall and sandstone roof about 1 break outby fall.

Acting as miner helper, watching cable when section of rib fell off striking victim
Repairman has hand in access hole checking to see if linkage needed repair. While troubleshooting, he had operator steer car. When steered, lever caught arm and injured left wrist. Returned to work next day with minor scrape on left wrist.

CLIMBING ACROSS SCOOP TO OTHER SIDE, FELL AND INJURED RIGHT KNEE.
WHILE CLEANING ROADWAY OUTBY LAST OPEN CROSS CUT RIB ROLLED INTO OPERATOR STATION INJURING HIM. PATIENT TOOK HIMSELF TO ER: PIKEVILLE METHODIST HOSP - RELEASED TO GO HOME.
