Miner crushed his right toes due to movement of cone while clearing rock jam. As he used pry bar while straddling cone last rock released stored energy of cone squeezing the side of his right foot.

Showing all 9The miner strained his lower back resulting in pain that required medical attention. This injury occured while the miner carried a generator weighing approx. 50lbs. up a set of stairs with six steps. The miner conducted this task by himself.
Miner was conducting maintenance work on front end loader bucket teeth. Miner was using a 3 lbs hammer to drive a pin when a metal fragment chipped off of tooth and became embedded into left bicep requiring medical attention.
Miner was attempting to climb a set of stairs into mobile control van, when his foot slipped off step, resulting in a twisting and hitting of his knee on step. Stairs have been replaced with new set providing a safer angle of rise and run.
Employee was holding bolt head with left hand while nut was being impacted off with an impact wrench. Employee cut left middle finger requiring 4 stitches.
Miner was injured by pry bar that was used as a guide for pulling a rope by a front end loader installing new feed belt on screen conveyor. Miner received contusion to left shoulder.
Miner was feeding pit run into crusher when cone crusher clogged requiring maintenance. Miner cleared clog using bar. A 4-5 in. rock was ejected hitting miner in face on left side in the nose and cheek area causing nosebleed and broken nose. Miner has knowledge and experience of pit conditions and process required to efficiently crush pit run.
Oper. moving material with loader and bottomed out seat, employee didn't seek medical attention at that time but did seek it 110905 was issued a return to work from the physician. He saw a Dr. on 111005 and again was issued a return to work with no restrictions. The he saw a Dr. on 112805 and was diagnosed with a bulging disk and was not allowed to return to work as of 112805.
Employee was on top of primary feed hopper on the jaw crusher unclogging lodged material when the bar he was using missed its target and traveled downward pulling his hand with it causing it to get smashed between the bar and grizzly bar. This accident was not caused or contributed by neither any rule nor regulation not being followed, or any used PPE.
