Miner crushed his right toes due to movement of cone while clearing rock jam. As he used pry bar while straddling cone last rock released stored energy of cone squeezing the side of his right foot.

Miner was feeding pit run into crusher when cone crusher clogged requiring maintenance. Miner cleared clog using bar. A 4-5 in. rock was ejected hitting miner in face on left side in the nose and cheek area causing nosebleed and broken nose. Miner has knowledge and experience of pit conditions and process required to efficiently crush pit run.

Employee was on top of primary feed hopper on the jaw crusher unclogging lodged material when the bar he was using missed its target and traveled downward pulling his hand with it causing it to get smashed between the bar and grizzly bar. This accident was not caused or contributed by neither any rule nor regulation not being followed, or any used PPE.

JAW FOR CRUSHER WAS BRIDGED. SCREEN DECK WAS SHUT DOWN. SMALL ROCKS WERE POURED INTO CRUSHER THRU THE BRIDGE TO FACILITATE CRUSHING. A 5"X10" ROCK WORKED BETWEEN FEEDER PLATE AND JAW RECIPROCA TING HOUSING. CRUSHING ACTION CAUSED THE ROCK TO WORK OVER GUARD NEAR BULL WHEEL. THE ROCK FELL INTO THE BULL WHEEL AND WAS THROWN BETWEEN THE FEEDER SHOOT AND GUARD STRIKING VICTIM APPROX 7'
