The miner strained his lower back resulting in pain that required medical attention. This injury occured while the miner carried a generator weighing approx. 50lbs. up a set of stairs with six steps. The miner conducted this task by himself.

Miner was conducting maintenance work on front end loader bucket teeth. Miner was using a 3 lbs hammer to drive a pin when a metal fragment chipped off of tooth and became embedded into left bicep requiring medical attention.

Oper. moving material with loader and bottomed out seat, employee didn't seek medical attention at that time but did seek it 110905 was issued a return to work from the physician. He saw a Dr. on 111005 and again was issued a return to work with no restrictions. The he saw a Dr. on 112805 and was diagnosed with a bulging disk and was not allowed to return to work as of 112805.
