Employee was using a rope to pull the hood open on the service truck. While pulling the rope, slipped causing the employee to fall backwards, landing on his rear-end.

Employee was walking when he stumbled on a rock and fell. He grabbed for the bumper on a truck and caught his right arm. As he fell with his right arm on the bumper, it caused him to dislocate his right shoulder. Reopened see DCN# 220101200018.EE is out having surgery.

Employee was changing a fuel filter on a CAT D10 dozer, when his foot slipped on the track, causing him to fall to the ground on his left arm. Jammed his arm around the elbow.

Employee was putting discharge hose on pump in pit. He cranked pump and it blew off the discharge hose at the pump. The hose struck employee in the chest.

EE had exited cab of 45 ton mobile crane & was walking on deck to outrigger to step down to the ground. He allegedly saw a spot of grease on the motor compartment & bent down to wipe if off. As he raised up he hit his head on the jib section which was folded up alongside of the boom. EE staggered & fell off the mobile crane to the ground below ( six foot fall).

Employee was cleaning coal in the pit, he got off of his trackhoe to move a pump hose. When he moved the hose it caused a rock to slide off the dig face and roll down and struck him in the lower left leg and foot.

When we arrived at the mine for the 1st shift, supervisor's found a D-10 dozer company # 221 had burned during the night. The evening shift had parked the dozer at 2:00 am. There were no signs of foul play.

Employee was walking through the parking lot, on his way to his vehicle, when he twisted his ankle. He went home and the pain got worse, he then went to the dr. and found out he had broken his ankle.
