Fall onto or against objects

Exiting truck when foot slipped causing employee to strike knee on step resulting in broken patella.

Employee was using a rope to pull the hood open on the service truck. While pulling the rope, slipped causing the employee to fall backwards, landing on his rear-end.

Employee was climbing down from the service truck when he slipped and twisted his left knee.

Fall from machine

Employee was changing a fuel filter on a CAT D10 dozer, when his foot slipped on the track, causing him to fall to the ground on his left arm. Jammed his arm around the elbow.

Struck by flying object

Employee was putting discharge hose on pump in pit. He cranked pump and it blew off the discharge hose at the pump. The hose struck employee in the chest.

Employee was getting off his service truck when he slipped and felt something pop in his left knee.

Struck by rolling or sliding object

Employee was cleaning coal in the pit, he got off of his trackhoe to move a pump hose. When he moved the hose it caused a rock to slide off the dig face and roll down and struck him in the lower left leg and foot.
