EE hand was on the side of piece of equipment. A rock fell from the top breaking right ring finger and also received 4 stitches

Employee bent over to pick a miner bit and felt pain in lower back
It had been raining that morning and haul road was wet and slick. The grader had not graded that section of haul road prior to the accident. As EE went around curve, the truck started to slide in the mud. Truck slid causing it to overturn.
The employee was pulling down a curtain that was hard to take down, when it came down, EE injured left shoulder, also causing pain in the neck, left arm and hand.
On 08/31/2017 employee was operating an end loader and ran over a rock jarring their lower back. Employee continued to work and on 10/25/2017 they went to the Doctor for treatment.
The employee was bending two roof bolts at the same time, and strained their shoulder.

Driver went into a curve and turned tractor/trailer over. Driver says does not know what happened.

EE climbed up on ladder of Komatsu 785 Haul Truck to inspect and secure a block heater receptacle that was broke off. When climbing back down off of the truck EE strained lower back. EE finished the shift and did not report injury till 1:00 pm on 7/25/2017.
Employee was on knees bolting, when ee twisted and felt left knee pop.
Employee was replacing grizzley flume screen when their hand slipped catching their right forearm on the rib on the grizzley causing a puncture to forearm. They received stitches and returned back work.

Employee was cribbing up the bumper of miner. The miner sit down on the end of the crib block, causing the other end of the crib to strike employee in the chin, causing a laceration.
A roof fall occurred in 2nd West Submain in #5 & #6 entries measuring 80 ft. long X 18.5 ft. wide X 9ft. high. This area was bolted with 6ft. fully grouted roof bolts and 9ft. cable bolts.
Employee was repairing a hydraulic hose on #146 1250 Komatsu excavator when EE slipped on spilled oil and fell on a compartment door. Injury was to the right rib cage area.
Employee was between #2 & #3 entries, employee was attempting to break a hose loose and when it did break loose employee fell backwards tripping over the bolter tray landing on employee's elbow, jamming employee's left shoulder.
The Employee stated: Employee was using a utility knife to cut a piece of ventilation curtain and cut left hand
The Employee stated: that EE was bending a roof bolt on a flat surface, the roof bolt twisted around while EE was bending it and EE felt a strain on the right side of EE's lower back.
Employee lifted a 7' timber twisted around felt a sharp pain in the middle lower back, resulting in a strain. Employee was at x-cut 25 on the Main North Supply Road.
Employee stated; was unloading 20ft sections of 4 inch fresh water line on #2 unit. Employee was working alone when they stated that they felt pain in their lower back on the left side.
3 individuals were in the process of replacing the lid on the power center. The injured person was on one side of the lid and the other two were on the opposite side of the lid. As they were placing the lid in place two of them accidentally dropped their end. The injured employees hand was underneath allowing the lid to land on left index finger.
Employee Stated: EE was building a brattice. EE turned around in a twisting motion and bent over, picked up a block and raised up turned around and felt a pain in lower back on the right side.
Employee chipped tooth on 8/16/2016. Was pulling pinner steel out of roof and hit self in the mouth chipping tooth. Employee went to dentist on 9/15/2016 to have cap put on therefore making this incident MSHA reportable.
EE States: EE was walking across a pile of gob in front of the feeder, and rolled ankle
Employee was holding a B/O hammer and another employee was hitting it with a sledge hammer driving the wedges out of Drag rope sockets on the bucket. Employee made a bad strike causing a piece of metal to fly off the B/O hammer and lodge in EE left forearm. Employee received 2 stitches in left forearm.
