Accident type, without injuries

A roof fall occurred in 2nd West Submain in #5 & #6 entries measuring 80 ft. long X 18.5 ft. wide X 9ft. high. This area was bolted with 6ft. fully grouted roof bolts and 9ft. cable bolts.

Over-exertion in lifting objects

Employee lifted a 7' timber twisted around felt a sharp pain in the middle lower back, resulting in a strain. Employee was at x-cut 25 on the Main North Supply Road.

Employee chipped tooth on 8/16/2016. Was pulling pinner steel out of roof and hit self in the mouth chipping tooth. Employee went to dentist on 9/15/2016 to have cap put on therefore making this incident MSHA reportable.

Employee was pulling a 6hp pump when EE slipped, fell catching another hanging cable, twisted hurting EE's lower back, and right hip.

A roof fall occurred in the 1st South Panel 2 at spad 4+00 in #5 entry measuring 40 foot long by 18 foot wide by approximately 9 foot high. The area was bolted with 9 foot cable bolts and 5 foot fully grouted bolts.

Struck against a moving object

Employee was driving to work in his personal vehicle on access road leading to mine. He ran into the mine's water truck on the access road. Employee suffered two cracked vertebrae in his neck.

Employee was going into the right return. When he opened the man door he was blown thru the door causing an injury to his lower back and right leg. This wasn't a lost time until 1/07/2015.

Employee was dropping his miner cable, when he raised up tie wire went between his glasses and scratched his eye.

Over-exertion in pulling or pushing objects

Employee was pushing on leveling bar with channel locks, with mechanic. When my back got tight, and started to hurt. When he went to stand up straight he had a sharp pain in my lower middle back.

Struck by flying object

Employee was driving a pin out of a ride with a hammer and chisel, when a piece of the chisel broke off and lodged under the skin on the left index finger. The employee received two stitches.

Employee was hanging continuous miner cable on #2 unit in #6 entry and strained his back.

Employee was cleaning out the pre-dump on the pinner because they had been having trouble with it clogging up throughout the day. While cleaning it out another worker turned the suction on and the flap closed on employee's hand causing the injury.

Employee filed report on 06/18/2015, stated he has a red rash on his forehead and believes the liner of his hardhat came into contact with glue used for installing roof bolts.

Employee stated he was operating a 2 man golf cart and hit a bump in the road, while traveling the 1D supply road causing a pain in his lower back. The employee did not miss work until 06-12-2015.

An unplanned roof fall occurred on Main North belt between spad 21+70 and spad 22+40 due to weathered roof, hidden slip. The fall measured approximately 45'long x 17' wide x 8' high.

The miner has participated in a medical examination authorized by the Federal Mine Safety and Health Act. The medical examination indicates that this miner has a sufficient degree of pneumoconiosis.

Employee (Roof Bolter Operator) was swinging in to begin installing the second roof bolt, when a rock slid off the canopy top striking his Right Ankle. Injured employee went to doctor on 04-13-2015.

Employee was shoveling on a belt line when the shovel came in contact with the moving belt causing the shovel to strike the employee above his right eye. He received six (6) sutures.

While employee was bolting #9 right a cable bolt that he was going to install that was laying on the bottom got hung up and he pulled the bolt he felt something pop in his left shoulder.

Employee was straining tore move a nut off of a bolt on a steering jack on a scoop when he felt a twinge in his lower back. This became a lost time injury on 4/28/2014.

While employee was walking to bolter in #8 entry he slipped and fell on right knee causing some swelling.

While employee was coming out of mines he went thru a set of metal airlock doors. As he went thru the doors they came back and struck him against the left leg upper thigh. A piece of metal was sticking out caught his leg causing a laceration.

Employee was handling a Roof Bolter cable when the cable reel started picking the cable up causing the cable to strike his left thumb. This did not become a Lost Time Accident until August 9, 2013.

A rock fall occurred on 3/11/13 in #5 entry approx. 70ft long x 19ft wide x 8ft thick in the 2nd West Panel off the 1st North Panel at 14+00. The fall was cribbed around.

When employee went to spin his cable bolt to mix glue a rock measuring 2 inches by 2 inches come out between the pie pan and top striking him on the upper left lip causing a laceration.

Employee was removing a wheel motor from a four man diesel ride when the motor fell striking the top of his left hand. The motor was being held by a hydraulic hose so it was a glancing blow causing a laceration to the top of hand.

Employee had made up his two bolts to put up and throwed them out on the ground. When he went to kneel down he caught the edge of a pin plate causing a laceration to the lower left leg.

employees were removing bolts on the end of a cutter drum using a torque multiplier and a long ratchet. There injured employee had set the multiplier on a bolt and put his hand on the equipment to stand. At this point the other employee pulled the ratchet down pinching the injured employees finger between the piece of equipment and the ratchet.

Employee was in process of removing a motor from a Damascus 2 man personal ride when it slipped out of the housing falling to the floor pinching two fingers between floor and motor.

Employee was cleaning out his bolter steel that was stopped up by beating it against the bolter head and caught his finger between the steel and the bolter head pinching it.

Employee was traveling underneath a belt line guard located on the 2nd Main Belt Line at crosscut 7+40 and stuck his head on the guard causing an injury to the back of his neck.

An area in 2nd East Submin #4 entry in pillar line 40+40 to40+90 some rock had fell out between some bolts and loosen up a couple bolts so the area was cribbed out to keep anyone from entering. When it was looked at on 04/10/2012 it had fell in some approx. 22'long,x 19'wide, x8'thick. On 04/03/2012 it was 16'long, x8'wide, x 4'high and was dangered out.

Employee was in the face of the #5 entry on the #1 unit assisting the roof bolter operator when his right foot was run over by the back tire as the machine was being trammed up to install another row of roof bolts.

Employee was roof bolting in the face of #8 entry on the #1 unit when a piece of rock, measuring approximately 3ft. long by 2ft wide x by 1 inch thick, fell from the mine roof striking his mid to lower back causing his elbow to strike the panic strip on the roof bolter. The became a Lost Time Accident on August 25, 2011.

The area was supported with 4 foot fully grouted roof bolts and 8 foot cable bolts.

Employee was operating a roof bolter and as he was drilling a hole, in the mine roof, the pinner steel stopped up. To unstop the drill steel he struck it with a pinner wrench. The pinner wrench slipped from his hand causing it to bouce backwards striking him in the right eyebrow. He received 7 sutures.

EE stated that while he was roof bolting on the left side roof bolter(installing 8 foot cable bolts), he felt something pull in his right shoulder. This caused severe pain while attempting to move his arm in the upper position. He was taken to the hospital and x-rays were taken. He is scheduled for a MRI at a later date. The doctor believes he has a damaged rotator cuff

The EE stated that he was in the process of roof bolting the face. While he was removing the drill steel from the roof and waiting to use the wrench, the steel came out, striking him on his middle finger on the right hand. He was taken to the hospital with a laceration near the knuckle. It took 5 stitches to close the wound.
