A roof fall occurred in 2nd West Submain in #5 & #6 entries measuring 80 ft. long X 18.5 ft. wide X 9ft. high. This area was bolted with 6ft. fully grouted roof bolts and 9ft. cable bolts.

Employee lifted a 7' timber twisted around felt a sharp pain in the middle lower back, resulting in a strain. Employee was at x-cut 25 on the Main North Supply Road.

Employee chipped tooth on 8/16/2016. Was pulling pinner steel out of roof and hit self in the mouth chipping tooth. Employee went to dentist on 9/15/2016 to have cap put on therefore making this incident MSHA reportable.

Finger caught between segment of vibrator screen and frame of vibrator as it was being removed.

Roof fall on high line 2nd East Panel 2 in number 4 entry at spad 10+ 20. 50' long x 18' wide x 5' to 8' high.

Rock fall was found on 1st North Submain1 intake in #8 entry at spad 1+80, 18' long 18' wide, and 9' high.

Employee was pulling a 6hp pump when EE slipped, fell catching another hanging cable, twisted hurting EE's lower back, and right hip.

A roof fall occurred in the 1st South Panel 2 at spad 4+00 in #5 entry measuring 40 foot long by 18 foot wide by approximately 9 foot high. The area was bolted with 9 foot cable bolts and 5 foot fully grouted bolts.

Employee was driving to work in his personal vehicle on access road leading to mine. He ran into the mine's water truck on the access road. Employee suffered two cracked vertebrae in his neck.

Employee was going into the right return. When he opened the man door he was blown thru the door causing an injury to his lower back and right leg. This wasn't a lost time until 1/07/2015.

Employee was dropping his miner cable, when he raised up tie wire went between his glasses and scratched his eye.

Employee was pushing on leveling bar with channel locks, with mechanic. When my back got tight, and started to hurt. When he went to stand up straight he had a sharp pain in my lower middle back.

Employee was driving a pin out of a ride with a hammer and chisel, when a piece of the chisel broke off and lodged under the skin on the left index finger. The employee received two stitches.

Employee was hanging continuous miner cable on #2 unit in #6 entry and strained his back.

Employee was cleaning out the pre-dump on the pinner because they had been having trouble with it clogging up throughout the day. While cleaning it out another worker turned the suction on and the flap closed on employee's hand causing the injury.

Wind blew warehouse entrance door open striking employee, breaking a tooth.

Employee filed report on 06/18/2015, stated he has a red rash on his forehead and believes the liner of his hardhat came into contact with glue used for installing roof bolts.

Employee stated he was operating a 2 man golf cart and hit a bump in the road, while traveling the 1D supply road causing a pain in his lower back. The employee did not miss work until 06-12-2015.

An unplanned roof fall occurred on Main North belt between spad 21+70 and spad 22+40 due to weathered roof, hidden slip. The fall measured approximately 45'long x 17' wide x 8' high.

The miner has participated in a medical examination authorized by the Federal Mine Safety and Health Act. The medical examination indicates that this miner has a sufficient degree of pneumoconiosis.

Walked behind bolter turned around back and knee popped.

Employee (Roof Bolter Operator) was swinging in to begin installing the second roof bolt, when a rock slid off the canopy top striking his Right Ankle. Injured employee went to doctor on 04-13-2015.

Injured employee stated he was loading glue into the tray when a rock (6""x6""x2""thick) fell mashing his right index finger.

Injured employee stated he was loading supplies onto the roof bolter and pulled a muscle in his lower back.

Employee was shoveling belt at the 1C Header, when he felt a pain in his back. He did not report this incident until 03/11/2015 and brought in a doctors excuse missing today (03/30/2015)

Operator of Ram Car ran over rock in road between #6 and #7 entry in last open, causing him to hit his head on canopy.

Tripped on Gob, twisting left ankle.

Injured employee was holding up a 3/4"" plywood door while hinges were being installed when his right knee gave out.

Employee cut into entergized cable resulting in shock and minor burns on left fingers.

Taking bolts off belt cable, when the tension released and the cable wrapped around his index finger and tried to take it off.

Employee states he was pulling up bolter cable from slack pile. When cable reel kicked in, causing the cable to strike him in the face, causing him to fall to the ground injuring his back.

A unplanned roof fall occurred in the pillar line between x-cut # 70 and # 71 of the 1st Main North Belt entry. Fall is approximately 13 feet long x 4 feet high x 10 feet wide.

He was tighten belt rope with a chain ratchet (come-along) rope dog slept and chain from ratchet came back and struck his thumb.

Employee was shoveling on a belt line when the shovel came in contact with the moving belt causing the shovel to strike the employee above his right eye. He received six (6) sutures.

An unplanned roof fall occurred On the North Main belt between x cut 33 and x cut 34. The fall measured approximately five foot wide x forty foot long x eight foot high.

He was lapping belt with a pair of channel locks and felt a pull with a light burn in his lower back. This was not reported to the safety department until 09/15/2014.

A unplanned roof fall occurred in the 3rd East Panel, Spad # 2+50, Entry # 4. The fall measured approximately 18'W X 18'L X 8' H.

Installing a (5) Five foot roof bolt, bolt twisted causing victim to fall forward straining his lower back.

A unplanned roof fall occurred on the Main North, entry # 5, spad 36+00, x cut 54. The fall is approximately 70' in length x 18' wide x 8' high.

An unplanned roof fall occurred on the #1 unit ( Second West Submain) inby the tail piece, Spad (10+70) Entry #8R to #9 Entry. The fall is approximately 35' in length x 20' wide x 8' high.

Right index finger pinched between two 6x6 crib blocks.

Pulling curtain off the head of the miner, felt pain in lower back.

While employee was putting one piece of steel(used to drill a hole) the other section fell out of the hole striking the top of the right thumb.

While employee was bolting #9 right a cable bolt that he was going to install that was laying on the bottom got hung up and he pulled the bolt he felt something pop in his left shoulder.

While employee was jerking down a wing curtain he felt something pop in his left thumb.

While employee was in process of taking down a backboard he stepped around to get out of the way and walked into apiece of angle iron catching him on the upper inner leg causing a laceration.

While employee was installing a pump motor on a Joy ramcar with a chain hoist. As he was moving the motor it slipped pinching his left pinky finger next to a bolt head causing a Laceration.

A rock fall occurred on2/24/14 in #6 entry at 3+00 in the 2nd North Panel measuring 25ft long x 18ft wide x 8ft thick. The fall has been cribbed around.

While employee was cutting nylon bands off of cable bolts the belt knife he was using struck his right leg in the calf causing a puncture wound.

Employee was straining tore move a nut off of a bolt on a steering jack on a scoop when he felt a twinge in his lower back. This became a lost time injury on 4/28/2014.

While employee was walking to bolter in #8 entry he slipped and fell on right knee causing some swelling.

While the employee was cutting CO cable the knife slipped out and went across the top of employees right thumb causing a laceration.

A rock fall occurred on 11/7/13 in #3 entry approx. 70ft long x19ft wide x 9ft high in the 1st East Submain at 4+30. The rock fall has been cribbed around.

While employee was coming out of mines he went thru a set of metal airlock doors. As he went thru the doors they came back and struck him against the left leg upper thigh. A piece of metal was sticking out caught his leg causing a laceration.

Employee was handling a Roof Bolter cable when the cable reel started picking the cable up causing the cable to strike his left thumb. This did not become a Lost Time Accident until August 9, 2013.

Employee was pulling his 8'pin steels out of top when he felt something pop or pulled in his left shoulder.

While employee was loading pin plates in scoop bucket he was twisting and felt a twinge in his lower back straining a muscle in lower back.

A rock fall occurred on 3/11/13 in #5 entry approx. 70ft long x 19ft wide x 8ft thick in the 2nd West Panel off the 1st North Panel at 14+00. The fall was cribbed around.

When employee went to spin his cable bolt to mix glue a rock measuring 2 inches by 2 inches come out between the pie pan and top striking him on the upper left lip causing a laceration.

Employee was removing a wheel motor from a four man diesel ride when the motor fell striking the top of his left hand. The motor was being held by a hydraulic hose so it was a glancing blow causing a laceration to the top of hand.

Employee had made up his two bolts to put up and throwed them out on the ground. When he went to kneel down he caught the edge of a pin plate causing a laceration to the lower left leg.

employees were removing bolts on the end of a cutter drum using a torque multiplier and a long ratchet. There injured employee had set the multiplier on a bolt and put his hand on the equipment to stand. At this point the other employee pulled the ratchet down pinching the injured employees finger between the piece of equipment and the ratchet.

While employee was pulling the roof bolter cable thru a brattice causing a concrete block to become dislodged falling and striking employee on top of foot fracturing it.

While employee was hanging a wing curtain in #6 entry on #2 unit he turned and struck his right ear on a damaged pie pan pin plate resulting in a laceration. He received 5 sutures.

Employee was cutting belt, knife slipped, came down cut his upper right leg, eight stitches on the inside, and seven stitches on the outside.

A rockfall occurred in 3rd West Sub Panel in #3 entry from 8+50 to 9+00. It measured 30ft.long,x19ft.wide,x 8ft. high. The fall was cribbed out.

Employee was in process of removing a motor from a Damascus 2 man personal ride when it slipped out of the housing falling to the floor pinching two fingers between floor and motor.

Employee was cleaning out his bolter steel that was stopped up by beating it against the bolter head and caught his finger between the steel and the bolter head pinching it.

Employee was traveling underneath a belt line guard located on the 2nd Main Belt Line at crosscut 7+40 and stuck his head on the guard causing an injury to the back of his neck.

A rock fall occurred in 2nd Southeast Sub main in #7 entry at 5+80 in intersection. It measured 50ft.long x 19ft.wide x 8ft.thick.This fall will be loaded out bolted and cribbed.

A roof fall occurred in the 2nd East Submain in #5 entry at 40+90 intersection. It measured 45ft. long x 18ft. wide x 8ft. thick. The fall was loaded out cribbed around and rebolted.

A rock fall was found in 1st South Panel off 1st East Submain in #9 entry at 12+00 to 13+00 in intersection. It measured 100ft. long x 19ft. wide x 8ft. thick. The area has been timbered and cribbed around.

A rock fall was found in 7th North Panel off the 2nd East Submain in #8 entry at 1+00 in intersection. It measured 8' Wide x 16' long x 5' high. This area was mined approx. 12/14/2011. The area has been timbered around.

An area in 2nd East Submin #4 entry in pillar line 40+40 to40+90 some rock had fell out between some bolts and loosen up a couple bolts so the area was cribbed out to keep anyone from entering. When it was looked at on 04/10/2012 it had fell in some approx. 22'long,x 19'wide, x8'thick. On 04/03/2012 it was 16'long, x8'wide, x 4'high and was dangered out.

A rock fall was found while checking pumps inby spad #0+50 to 1+00 in #7 entry measuring 30ft. long x 18ft wide x 8ft high. Area was supported with 4ft fully grouted roof bolts, 8ft cable bolts and some cribs. This area was mined in October 2011.

Employee mashed 2 fingers on right hand between cab and bed of battery hauler causing fractures to the 2 fingers.

Guard on belt take up slipped while installing and impacted workers fingers.

While employee was taking lil-mac down slope it picked up speed and he couldn't slow it down so he tried to get off. He lost control and fell out on chest.

A roof fall occurred in the 1st East Submain in the #6entry at spad #19+40 measuring approximately 20ft. long x 18-19ft wide x 8ft.high. This area was supported with 4ft. fully grouted roof bolts and 8ft. cable bolts.

A roof fall occurred in the 1st East Submain in the #7 entry at spad #40+40 measuring approximately 30 ft. long x 18ft. wide X 9ft. high. The area was supported with 4ft. fully grouted bolts and 8ft. cable bolts.

While the employee was tieing up bolter cable in last open intersection in #3 entry on #2 unit he received an electrical shock to the back of his hands.

A roof fall occurred in the 1st East Submain off the Main North in the intersection of #3 entry at spad number 39+90 measuring approximately 19ft. long x 19ft. wide x 8ft. high. This area was supported with 4ft. fully grouted roof bolts and 8ft. cable bolts.

A Roof Fall occurred in the 1st East Submain off the Main North at spad number 48+90 in the intersection of #5 entry and extending through the pillarline between #4 and #5 entries measuring approximately 35ft. long x 17.5ft. wide x 8ft. high. The area was supported with 4ft. fully grouted roof bolts and 8ft. cable bolts. The area was mined in September 2011.

A roof fall occurred in the intersection of the 1st East Submain at spad number 21+90 measuring approximately 20ft. long x 19ft. wide x 8ft. high. This area was supported with 4ft. fully grouted roof bolts and 8ft. cable bolts. This area was mined in April 2011.

Employee was in the face of the #5 entry on the #1 unit assisting the roof bolter operator when his right foot was run over by the back tire as the machine was being trammed up to install another row of roof bolts.

Employee was roof bolting in the face of #8 entry on the #1 unit when a piece of rock, measuring approximately 3ft. long by 2ft wide x by 1 inch thick, fell from the mine roof striking his mid to lower back causing his elbow to strike the panic strip on the roof bolter. The became a Lost Time Accident on August 25, 2011.

A roof fall occurred in the 1st East Submain in the #3 entry measuring approximately 25ft. long x 18ft. wide x 8ft. high. This area was supported with 4ft. fully grouted roof bolts, 8ft. cable bolts and square cribs.

While employee was in process of removing a boomer off of a slinger duster on a scoop bucket the duster shifted causing the boomer handle to pinch the right small finger. He received 7 sutures.

Employee was drilling a hole when a piece of rock (slate) fell down spinning around steel breaking up into pieces striking the employee above the left eye causing a laceration. He received 2 sutures

A roof fall occurred on the #2 unit in the intersection of #8 entry at spad number 45+10 and extending through 1/2 the pillar line between #7 and #8 entries measuring approximately 25ft. long x 18.5ft. wide x 8ft. high. This area was supported with 4ft fully grouted roof bolts and 8ft. cable bolts.

A roof fall occurred in the 1st West Panel off the Main North in 1/2 the intersections of #5 and #6 entries at spad number 27+90 and extending through 1/2 the intersections of spad number 28+40 measuring approximately 70ft. long x 19ft. wide x 8ft. high. The area was supported with 4 foot fully grouted roof bolts and 8 foot cable bolts.

Employee was operating a roof bolter and as he was drilling a hole, in the mine roof, the pinner steel stopped up. To unstop the drill steel he struck it with a pinner wrench. The pinner wrench slipped from his hand causing it to bouce backwards striking him in the right eyebrow. He received 7 sutures.

A roof fall occurred in the 2nd West Submain in the #3 entry between spad number 27+70 and spad number 28+40 measuring approximately 34 ft. long x 19 ft. wide x 9 ft. high. The area was supported with 4 ft. glue bolts, 8 ft cable bolts and square cribs.

A roof fall occurred on #1 unit MMU 001-0 in the intersection of face area of #3 entry measuring approx. 16ft. long X 18ft wide X 7.5ft high. The area was bolted with 4ft. fully grouted bolts and 8ft. cable bolts.

A roof fall occurred on the #1 unit (1st West Panel) in the #9 entry in the intersection of spad number 17+50 and extending outby through 2/3rds of the intersection of spad number 17+00 measuring approximately 70ft. long x 19ft. wide x 6ft. high. This area was supported with 4ft. fully grouted roof bolts and 8ft. cable bolts.

Employee was cutting off a piece of rock dust hole using a utility knife when the knife slipped striking his index finger on his left hand causing a laceration. He received 6 sutures.

A roof fall occurred in the Main North in #2 entry starting inby the pillarline of crosscut 32 and extending through the intersection of crosscut 35 then extending through the pillarline of #3 entry measuring approximately 250ft long x 19ft wide x approximately 8ft high. This area was supported with 4ft fully grouted roof bolts and 8ft cable bolts.

A roof fall occurred in the 2nd East Panel in the #1 entry in the intersection of spad number 8+00 measuring approximately 20ft. long x 17ft. wide x 8ft. high. This area was supported with 4ft. fully grouted roof bolts and 8ft. cable bolts.

A roof fall occurred in the 2nd West Submain at spad number 5+90 in the pillar line between #8 and #9 entries measuring approximately 25ft. long x 18ft. wide x 7ft. to 8ft. high. This area was supported with 4 foot fully grouted roof bolts, 8 foot cable bolts and square cribs.

A roof fall occurred in the 1st South Panel in #5 entry in 1/3 the intersection of spad number 0+50 and continuing to the inby edge of spad number 0+150 measuring approximately 90ft. long x 19ft. wide x 8ft. high. This area was supported with 4 foot fully grouted roof bolts.

A roof fall occurred in the East Mains in the intersection of crosscut 23 in #2 entry measuring approximately 25ft. long x 18ft. wide x 8ft. high. This area was supported by square cribs, 4ft. fully grouted roof bolts and 8ft. cable bolts.

EE stated that while he was roof bolting on the left side roof bolter(installing 8 foot cable bolts), he felt something pull in his right shoulder. This caused severe pain while attempting to move his arm in the upper position. He was taken to the hospital and x-rays were taken. He is scheduled for a MRI at a later date. The doctor believes he has a damaged rotator cuff

A roof fall occurred in the East Mains in the intersection of crosscut 7 in the #2 entry measuring approximately 25ft. long x 18ft. wide x 9ft. high. This area was supported by square cribs, 4ft. fully grouted roof bolts and 8ft. cable bolts.

The EE stated that he was in the process of roof bolting the face. While he was removing the drill steel from the roof and waiting to use the wrench, the steel came out, striking him on his middle finger on the right hand. He was taken to the hospital with a laceration near the knuckle. It took 5 stitches to close the wound.
