Showing all 4Employee was pulling chain onto dredge pontoon for repair. Chain got caught on a bolt head of pontoon access panel. When chain popped free, the resulting slack caused employee to slip backwards into the hand railing of the steps built over the lift winch.
Was pulling on a safety cable out of the water to attach to the dredge, when EE felt a pop in the right shoulder. EE went to hospital where it was determined that EE strained the shoulder.
Employee was using a 5" portable grinder with a metal cutting wheel to cut a slot in a bracket when the grinder kicked back into him.
