A roof fall was discovered in at Break 20, 3 Breaks inby SS #64 in the D main line return, No. 1 entry. This fall does not affect ventilation in any way. This fall has occurred in an intersection of a break. The area of concern was investigated by MSHA & company official. The area has been secured off by cribbing & timbers.

The employee was using a crescent wrench to tighten some blots when the wrench slipped and his right hand came in contact with a piece of expanded metal. The sharp edge of the metal cut his right hand around his knuckle area.

EE was using a Johnson Ride to pull a pull behind rock duster he said he was backing up and hit the Head Drive #4 said it made him hit his back against ride he was operating. EE stated foot switch stuck on him. (maint. checked foot switch and found no problems)

EE was helping install belt when the chain broke that was supporting tail piece, causing EE to mash his thumb between the anchor pin and turn buckle. Injuries were a broken right thumb.

EE was the scoop operator on the 001 section on 10/16/12. The foreman found ee unconscious beside the scoop face down. EE had no apparent injuries and the area appeared to be safe to work and travel. This was a medical condition that caused this incident.

EE hit his knee on a Johnson 3 Wheeler on July 2/, 2012. He refused medical attention at this time. On August 6, 2012, he came forward & requested to go to Doctor, we took him to Dr. He was given (1) day off. He was off work 8/7/12 and returned to work 8/8/12.

Employee got out of his vehicle, took a couple of steps and tripped on his loose shoe strings and fell. He tried to catch himself with his left hand, causing all his weight to shift to his left hand and arm. Nature of illness, broken wrist.

EE was operating 2-man Johnson - stinger ride when he hit a hole in the bottom, causing the steering wheel to turn quickly. The ball on the steering wheel hit him on the back of his hand, breaking a bone.

EE was drilling a hole for his roof bolt when the drill steel broke, throwing a piece of steel, striking him in the mouth. He required 8 stitches on his lip and he broke a tooth, which will require dental work.

EE was changing a boom jack on a fletcher bolt machine when he lost his grip on the jack. Jack fell on finger catching it between jack and frame of bolt machine, injury caused laceration on left hand ring finger,(8) eight stitches were required to close laceration.

A roof fall occurred at approx 2:30am 9/14/11. EE 3rd shift mine foreman and his crew was installing belt in the D-mains panel belt line when he heard a roof fall. Upon his investigation he found a roof fall in the A-mining return. He immediately got his crew together and went outside and reported the fall to MSHA

IE was assisting another EE make a belt splice. IE was holding the nail board in place while the other EE was striking it with a hammer. IE's finger slipped sideways as other EE was hitting the nail board, striking IE's finger.

EE was assisting other ee, putting in belt structure in the main line belt line. There was no electrical power going U.G. at this time. A severe electrical storm was in progress outside, apparently lightening traveling by way of the bed railing structure as ee came in contact with it underground.

Employee was using an air greaser gun greasing a 980G Cat loader, the hose busted cutting his right hand next to his index finger. Grease entered the wound. He was taken for medical treatment to clean the cut.

While installing a roof bolt, a spur on the bolt caught the EE's glove as he started his rotation causing his thumb to twist. He finished his shift and worked the next 8 shifts before consulting a doctor. He told his foreman that he was fine and did not require attention. On 5/2/11 he informed us that he had consulted a doctor.

Rockfall was discovered on 12/21/10 in the 1st right mains #4 entry on the intake neutral. Rock was approximately 7'-8' high, 50' long and 19' wide. No ventilation was disturbed , no injuries, no equipment was involved, fall has been timbered and dangered off.
