Employee was helping mechanic, said his back started hurting. The employee did not report this until 3-3-12. Went to the doctor started missing time on 3-4-12. Returned to word on 3-8-12.

Employee was using an air greaser gun greasing a 980G Cat loader, the hose busted cutting his right hand next to his index finger. Grease entered the wound. He was taken for medical treatment to clean the cut.
Employee was unbolting cutting edge from a 988F Loader, as he was removing the last bolt the cutting edge popped loose, dropped onto to his left foot.

The EE stated he was starting to climb on the machine after finishing his pre-inspection and his foot slipped off of the bottom step and hit the ground, injuring his back. The accident was not reported till the end of the shift. This was a non-lost time accident until 11/27/06. The EE went for test because of back pain. Restricted duty as of 11/28/06
Employee was loading blasting supplies out of a box trailer into a pickup truck. When finished he stepped out of the trailer onto the tailgate of the truck and his foot slipped off the tailgate causing him to fall back against the trailer landing on his rt hip.
Greaser was operating a 8000 Ford lube truck returning to parking area at quit time traveling down a App. 6' roadway and stated his brakes went out. He jumped from truck landing on gravel road and fell. He had minor scrapes on R elbow & R side sprained his L ankel. Truck went approx.100' & went up on berm & stopped.

WHILE OPERATING AT 777 CAT HAUL TRUCK, THE DRIVER WAS DUMPING LOAD OF ROCK FROM THE TRUCK. DRIVER RAISED THE BED AND LOAD DID NOT ALL COME OUT OF TRUCK BED. DRIVER THEN PUT THE BED LEVER IN DOWN POSITION AND BED FELL 3 OR 4 FEET JARRING THE OPERATOR.
The greaser was helping the mechanic put new teeth on a 992-G loader bucket. While driving one of the old pins out with a hammer, a small piece of metal chipped off of the pin and went in his leg near his knee.
EE was operating a fuel service truck. Equipment #FT01 - an 8000 Ford - Said accident happened when servicing #HL01 92 Cat Loader. After fueling loader EE was climbing off loader when he struck his right hand against the handrail, breaking right hand.
Hyd. hose end came off of a hose going to head drive on the mast of the drill, letting the hose strike the left side glass of the operator's cab, and broke the glass. The operator stated he was ok. He decided to go to Dr. the next shift to get his eye checked.

Employee had climbed up on dozer with fuel hose to fuel dozer. When he started to remove fuel cap he said a strong gust of wind caused him to lose his balance causing him to fall to the ground, about 11'.

EE WAS OPERATOR ON A EXCAVATOR, DIGGING MATERIALFROM THE BACK OF THE SHOT AT THE WALL. A ROCK WEIGHING 50-60 LBS FELL FROM THE HIGHWALL ABOVE THE EXCAVATOR HITTING THE FRONT EDGE OF THE CAB ABOVE THE WINDSHIELD. STRUCK OPERATOR ON THE HEAD AND RIGHT SHOULDER.

EE WAS MOUNTING CAB OF D25 WATER TRUCK. HE STEPPED UPON 1ST STEP & LOST HIS HOLD ON GRAB IRON WHEN REACHING FOR THE OTHER GRAB IRON ON DOOR. HE LANDED ON GROUND ON HIS FEET LATER THAT DAY HE STATED THAT HIS BACK WAS HURTING HIM
EMPLOYEE HAD JUST FILLED D8R DOZER WITH FUEL. HE DROPPED FUEL NOZZEL TO GROUND AND TURNED TO COME OFF DOZER. HE SLIPPED & FELL TO GROUND INJURING HIS LOWER BACK. HE FELL ABOUT 7 FT & 6 INCHES.
