A roof fall was discovered in at Break 20, 3 Breaks inby SS #64 in the D main line return, No. 1 entry. This fall does not affect ventilation in any way. This fall has occurred in an intersection of a break. The area of concern was investigated by MSHA & company official. The area has been secured off by cribbing & timbers.

EE was helping install belt when the chain broke that was supporting tail piece, causing EE to mash his thumb between the anchor pin and turn buckle. Injuries were a broken right thumb.
EE was walking behind the bolt machine when he tripped over a rock and twisted his knee.
Employee got out of his vehicle, took a couple of steps and tripped on his loose shoe strings and fell. He tried to catch himself with his left hand, causing all his weight to shift to his left hand and arm. Nature of illness, broken wrist.

EE was pulling miner cable out of roadway, so the roof bolter could get by, when he felt a sharp pain in his lower back.
EE was changing a boom jack on a fletcher bolt machine when he lost his grip on the jack. Jack fell on finger catching it between jack and frame of bolt machine, injury caused laceration on left hand ring finger,(8) eight stitches were required to close laceration.
A roof fall occurred at approx 2:30am 9/14/11. EE 3rd shift mine foreman and his crew was installing belt in the D-mains panel belt line when he heard a roof fall. Upon his investigation he found a roof fall in the A-mining return. He immediately got his crew together and went outside and reported the fall to MSHA
EE was assisting other ee, putting in belt structure in the main line belt line. There was no electrical power going U.G. at this time. A severe electrical storm was in progress outside, apparently lightening traveling by way of the bed railing structure as ee came in contact with it underground.

Rockfall was discovered on 12/21/10 in the 1st right mains #4 entry on the intake neutral. Rock was approximately 7'-8' high, 50' long and 19' wide. No ventilation was disturbed , no injuries, no equipment was involved, fall has been timbered and dangered off.
Employee alleged pain in his back as he was unloading block. He finished his shift and did not report the alleged injury until a later date.
EE became ill at work and he was transported to the surface and was transported via a waiting ambulance to Wellmont Hospital at Big Stone Gap, VA and then Med Flighted to Bristol Regional Med Center, Bristol, TN, where later he died from a cerebral hemorrhage-stroke.
Employee was watching the belt & had his hand on the belt rope & a small piece of draw rock fell & mashed the end of his ring finger on left hand. Tip of left finger was amputated by doctor.
As EE did a belt fire boss run at 6 left drive he slipped and fell hurting his left knee.

EE dropped a knife, he slipped in mud and fell on the knife.
Water inundation. The water entered through the bleeder system in the A-side return.
EE states he was cutting straps on roof plates and the utility knife blade game out and cut ee on his right hand between his thumb and finger.
EE states that he hurt his back when he lifted two (2) buckets of miner bits. Employee returned to work on 8/16/10 & worked 8/16-19/2010. He called us on 8/24/2010 telling us he was back off work because of this injury (4-7-10) - No new accident.
Employee was at beltline drive and turned around and twisted his right knee. Employee stated that he was standing in mud at the time of injury. Employee began missing work on 4/8/2010.
On 6/17/10 employee reported to mine management that on 3/18/10 he stepped backward off the scoop and twisted his left knee. *MRI has been rescheduled twice and he is still waiting on an MRI. No missed time as of 8/3/2010.
EE stepped on a 4"" water line and fell. EE did not go to the doctor until 1/17/10. He began missing work on 1/18/10
Employee fell during snow storm and hurt his right elbow.

Employee got out of scoop and said he hurt his lower mid section.
Employee was standing in the shop, when he was hit by a 4 wheel ride. * 1/18/10: EE returned to work on 1/17/10. Worked one shift and part of second. Alleged he could not work without hurting his shoulder.
EE stated he slipped and fell, hurt his back.
EE was riding outside on the mantrip, when a piece of little drawrock fell and hit his safety glasses and his safety glasses caused a skinned spot on the left side of his nose. It was cleaned and a band-aid put on the scrape. Did not go to the doctor.

Roof fall in #4 panel. 20'x20'x7' area was timbered and dangered off.
Employee threw a derail & the employee did not remove his hand & caught his middle right finger between the derail & track.
Roof fall in #4 heading, approximate size is 15' X 20' X 5'.
While roof bolting, Employee dropped roof bolter head and pushed pinner steel into and thru his left thigh.
Employee was putting feeder chain together. He was putting chain keeper in chain with a hammer when the head of the hammer busted causing it to fly off. A piece of steel hit him in the left knee.
EE drove the ride 2000' with a flat tire and melted the battery down and caught the ride on fire.
Employee was operating the scoop when a piece of coal came in inside (sideways-because canopy was on scoop) and struck employee behind right ear. 3 stitches required
Employee was standing in intersection when pinnerman turned on reel. The pinner cable jerked employee's legs from under him causing the employee to hit his head on the right rib.
Employee was making a belt splice & caught his right ear on a roof strap. No stitches - glued laceration in ER.
Cut right hand with knife. Trimming belt knife blade broke.
Employee was cleaning belts, slipped and hurt right knee.
Employee was installing a roof bolt. While he was spinning the roof bolt and plate, a small piece of rock spun off the plate and struck employee in the lip.
Employee was running centers & slipped on the wet bottom & twisted his left knee.

After loading his shuttle car, ee put his hand on the hand hold mount to the canopy of the shuttle car. As he started to move the car, a roof bolt plate spun around and lacerated his finger.
Pinion shaft broke in rear end - the ride lost brakes, one man hurt right leg and neck.
Employee caught his finger between pinner head and plate. Broke end of finger and laceration with stiches.
Employee was changing a scraper on #4 belt; when he took it off and trimmed it, the knife slipped and cut his upper right leg.
Building trussel, when track nail in board ripped inside right hand.
On 003 Section while pillaring, test holes checked by foreman, an unscheduled slip occurred that unintentionally fell and covered miner & shuttle car. No injuries. Equipment unrecoverable.
Employee prior to shift start was proceeding up snow covered steps when he fell and hit left side of chest on the steps.
Employee is a service man on equipment underground. Another worker dropped a tool box lid on his finger (left index finger) while changing nitrogen tank.

EMPLOYEE WAS WORKING ON FEEDER AND WHEN HE PUSHED A LEVER, A HYDRAULIC HOSE BROKE AND HIT EMPLOYEE IN HIS RIGHT EYE.
Employee was riding out of mine on mantrip and in a low spot he raised his head and a roof bolt knocked off his hard hat and the next roof bolt cut his head. Laceration to left side of top of head requiring 6 staples.

WATER BUILT UP BEHIND A SET OF SEALS AND #5 SEAL BROKE THROUGH AND WATER RAN TO OUTSIDE IN #3 PORTAL. NO WATER WENT TO ACTIVE SECTION. ALL U.G. POWER WAS DENERGIZED AND ALL MEN REMOVED FROM MI NE BY 8:20 AM 0.2-0.3 CH4 WAS DETECTED @ #6 SEALONLY MSHA WAS NOTIFIED AT 8:30 AM.
