Employee was mounting a CAT 980 Loader when EE's foot slipped on the the step causing EE to catch self with EE's left arm. EE was diagnosed with a torn tendon and muscle and had to have surgery on 5/14/2026. EE's first day missing work was 5/14/2026.

As they were reaching under the bucket to get the bolt, the sharp edge of the bolt cut the palm of left hand. Eight (8) sutures were required to repair the wound. The employee returned back to work on the same shift and finished the shift. Employee was returned back to work full duty.
Employee was walking from EE's private vehicle to EE's rock truck. As EE was walking across the parking at the truck ditch, EE slipped and fell on the snow and ice. EE was diagnosed with a fractured femur and started missing work on 1/11/2025
Employed jumped from a rock truck that caught fire and fractured right heel.
The miner was changing a tire on a Cat 789 rock truck when EE's finger was caught between the locking ring and bead ring. The tip of EE's right ring finger was smashed causing the tissue and skin to be removed from it.
Employee was attempting to install the rear belly pan on a D10T dozer. Employee had the belly pan supported with a chain that was draped over a cross member to hold the belly pan. As employee was lining bolt holes up with a pry bar one corner came unhooked causing the pry bar to hit employee in the left wrist.
Employee was trying to break up some lodged anfo in the bed of a bulk anfo truck while employee was using a shovel employee had a sharp pain travel through shoulder and neck area. Employee started missing work on 8/12/20.
Employee and another employee were closing a garage door and the cable broke causing one of the rollers to come out hitting the employee in the head requiring 2 staples.
Employee was starting equipment prior to other employees running the equipment. EE had started a D11 dozer and when EE stepped off the platform onto the track, EE's foot slipped off the wet track causing left ankle to roll to the side causing a severe strain.
Employee was coming down hill and knocked the truck out of gear and could not get it back in gear. Employee jumped out of the truck breaking left ankle and causing a laceration to the back of the head.
Installing a hydraulic hose on main frame of 16 grader when lower back popped and EE felt a stabbing pain

Employee was installing an Equalizer E-Bar pin on a D11R, Unit #3121, at location. EE was installing the left side using a hammer when a piece of the pin broke off striking them in the left leg below the knee. Went to ER for foreign body removal. Given antibiotics for infection prevention.
Knocking a pin out of ebar and ebar fell pinning finger against the belly pan of the machine lacerating finger
Employee stated was backing up with the grader and did not see the rock truck due to the standard inside the cab of the grader. The front of the rock truck caught the ripper arm of the grader spinning the grader around and hitting the back tire of the rock truck. This caused EE to fracture 3 ribs.
EE haulage truck was transporting coal to typo tipple on haul road at east long mine when ee truck was struck in the side from behind by another ee truck driven. The other ee jumped from the truck making it a runaway because it was said it would not stop
Driver lost control, jumped from moving, loaded vehicle. Broke both feet.
Employee was dismounting a D-11 Dozer and as employee stepped on the last step next to the ground, something "popped" in right foot.
Employee was opening the inspection door on the sampler system in the load out building and it struck employee's left index finger and cut it. Resulted in 5 sutures and employee returned back to work next scheduled shift.

Employee was cleaning mirrors and dropped cleaner on the ground. When he was climbing back up the end-loader his foot missed the top step and he slipped falling to the ground. Started missing work on September 11, 2015.
Backed up on a rock with a dozer and hit it hard with track. Jarred him and said he hurt his neck. Did not wish to go to doctor and didn't want medical attention. Went to see his family doctor after work and said he had a pulled muscle in his back.
Victim was climbing up ladder on 785B truck when he hit his knee on button of step. Made a small incision across knee cap about 1/2 to 3/4 long. Because it was in bend of knee, would not close. Taken to hospital for stitches.
