Climbing up to put oil in the water scraper, EE's left foot got caught on the lip at the top. EE stepped up with EE's left foot and was bringing up EE's right foot and EE's left foot slipped. EE had a hold of the hand rail and tried to catch self, EE's right foot got caught on something causing the injury.

Showing 50 of 89EE finished greasing the crusher and was climbing down and didn't notice the cross bar and stuck EE's hand thru it and as EE was stepping down EE's index and middle finger on EE's right hand were pinched.
Employee was loading a truck with 200 lbs rip rap and one boulder fell off and they didn't notice it. They pulled forward running over the top of the boulder, that caused them to jar their back. Went back to work but as the day progressed so did the pain in their lower back and they notified their supervisor.
Was shutting off water pump after filling the water tuck. Was walking back to the truck, stepped on a rock, which caused them to stumble, they felt a pop and had some pain. Told a supervisor about it, thought they had sprain an ankle. As the day went on the pain became worse and they sought medical treatment.
Was shoveling out mud out of the secondary crusher when the shovel hit the inside of the lip on the crusher causing the shovel to stop abruptly and bending their right wrist back causing immediate pain.
Employee was climbing a ladder to access a screening plant when the ladder moved. EE reached out to grab the chain to steady self and heard a pop in EE's shoulder.
Was putting a drive belt on the crusher and was checking the underside to see if the belt needed to be rotated and the belt was put into reverse and EE's hand was on the belt and was pulled in between the belt and shiv, injuring EE's left hand.
Was driving a haul truck when the truck in front of EE stalled. EE got out to see if EE could help. EE climbed up on the catwalk when the brakes disengaged and started rolling backwards. EE jumped off the haul truck and in doing so strained calf muscle and upon hitting the ground strained left wrist.
Was hauling boulders to the crusher, EE got out of truck to move a rock that was in the middle of the road. EE picked it up and dropped it on right foot.
Was passing another truck on the haul road and they got over on the road a little too far. The road was soft due to rain and they started to slide off the road. They stopped and opened the cab door to access the situation and the cab door shut on their right ankle, breaking the ankle.
Was stepping down off of a piece of equipment, had one foot on the step and one foot on the track of the loader. The foot on the track slipped and their Left knee popped.
Was working on scaffolding, was climbing a ladder from the ground when the ladder tipped and they fell into the ladder on the way down. They tried to grab the X bar on the scaffolding and missed, they landed on the ladder hurting their back and neck.
Miner was in a skid loader on a designated route. A haul truck was headed toward the miner. It appeared that the haul truck driver did not notice the skid loader. Skid loader operator turned and was hit by the haul truck and spun out the skid loader, the skid loader operator hit their head when the skid loader came back down, hit their head again and also injured left shoulder.
Individual was exiting the Komatsu 220 excavator and missed the step. At which point EE's left foot was on the track and right foot was on the ground. EE did not fall, nor did EE bump knee. EE did however over extend the left knee.
Couple of boulders got stuck in the feeder. Employee walked up to catwalk to take a look and a rock shot out of the feeder hitting them in the chin on the right side causing a laceration.
Employee was getting out of a skid loader. EE's pant leg got caught on one of the latches as EE was getting out causing EE to fall and hit R knee. Went to K-Stat had x-rays, told EE to put ice on it and released EE with no restrictions.
Employee was climbing down off the loader, tried to turn around and EE's knee gave out and EE fell to the ground. Left knee is the injured area.
Was loosening a high pressure hose on a piece of equipment and thought they had released the pressure on the hydraulics. When the hose was loosened it broke loose shooting hydraulic oil onto EE's hard hat and then on the right side of EE's neck. Checked self out and there wasn't any broken skin. Went to urgent care next day to be checked out.
Miner was shoveling off the cat walk and was twisting to throw it off the machine and felt it in EE's back.
It had been freezing rain over night and in the morning. The employee was headed to the facilities from the scale house. EE stepped off of the steps to the ground and EE's feet came out from under EE and EE fell, breaking EE's ankle.
Was operating a front end loader to load the shoot. A boulder became dislodged & obstructed EE's path to the shoot. EE got out of the loader, was pushing a boulder out of the way so EE could load the shoot. Pushed on it & it wasn't enough force. EE lowered stance & when EE pushed the boulder again EE's right index finger got pinched between the boulder & another rock.
Operator was drilling a hole for setting off a shot. Operator looked to the left and noticed the dirt moving, felt the ground tremble a little and looked up to see the dirt from the ledge above falling. It crashed into the equipment pushing operator to the right and burying operator up to waist and pinning left leg. Two of our employees ran over and dug the operator out.
Was opening the doors on a Conex Box and the doors bound up. Employee was trying to get the doors open and right shoulder popped.
Was loading a screen into the loader bucket, was not wearing gloves and got finger caught on the edge of the screen cutting right index finger.
Was walking to a skid loader when ee tripped and stumbled, tried to catch self and fell into the spoon attachment on the skid loader.
Was operating a haul truck with an exhaust leak for a day and a half before EE realized what was going on. EE was put in a different truck and the mechanic was called to repair the truck. The accumulative affects hit EE on Thursday on the way into work and EE went to the emergency room.
Had a bar stuffed up in the bin trying to dislodge a boulder. The bar slipped and ee jammed left shoulder.
Was unhooking a chain on a conveyor. Thought the conveyor was on the ground but was hung up on something. The conveyor released from whatever was keeping it up and dropped on right foot.
Was installing a stick cylinder on the 600 excavator,CS60. Was removing the lifting strap from around the cylinder, laid left arm on steel hydraulic line so they could use both hands. When they stood back up drug their arm across a burr on that line, slicing open the left arm from the wrist to the forearm.
Was working on a scalping screen removing a bolt with right hand. Employee turned with shoulders to hold the bolt with the left hand when their back popped.
EE was helping a co-worker remove a guard from Q-173 portable crusher and the guard slipped and fell hitting EE's right shoulder.
Was pulling a 20 ton floor jack out the back of truck when ee dropped it and it landed on left foot just behind the steel toe causing a contusion.
Was climbing into a skid loader when EE slipped on lime dust on the metal step plate. EE tried to catch self but right knee hit the metal step plate.
Was kneeling on gravel working on a conveyor and been kneeling for an extended period of time and when EE went to get up experienced sharp pain in the shins.
An excavator over heated and ee turned the radiator cap and the steam was released and burnt left hand and forearm.
While driving an articulating truck at the quarry, something fell into EE's left eye.
EE went to use the facilities and when EE returned EE was going to clean off shoes with the mud jack, when EE lifted foot the other one slipped out from under EE causing EE to fall on back hurting left hip, right knee, left shoulder and head.
EE was making hangers to hold electrical cords for the crusher. EE was punching holes thru the rubber and hit left thumb with the hammer, causing a contusion.
Was closing the passenger side door to the haul truck EE was about the enter and EE closed the passenger side door on EE's right thumb crushing EE's thumb.
Was servicing piece of equipment when the radiator door strut on the skid loader failed and came down on left forearm, with some velocity.
Miner was attempting to start a pull start pressure washer, when he pulled on the rope and released the rope his left index finger got caught in the rope and pulled the skin back between the last and middle knuckle.
Individual was climbing out of the water truck to turn off the PTO pump and tripped on the fuel cap causing him to hit his knee on the ladder of the truck.
The miner was working outside in the heat. He entered the scale house complaining of being hot. He drank some water and complained of nausea and said he had stopped sweating. He was taken to K-stat to be checked out.
Was working on conveyor belt cutting the conveyor belt when the knife slipped cutting right palm of hand.
Employee was closing the entrance gate to mine and slipped on snow/ice. Employee strained upper leg(thigh.)
Was helping to dislodge a rock wedged in the tailgate of a dump truck. When the rock fell out the tailgated slammed shut on the second finger of the left hand cutting the finger.
He was putting a V belt on the shive, when it rocked on his thumb got pinched.
Miner was hanging a power cord and while twisting his body felt a twinge in his back. Finished hanging cord and reported to supervisor. Supervisor offered to take him to medical facility and miner refused. Supervisor then sent miner home to rest his back the rest of the day. Next morning miner called and said he should seek medical attention. He was then taken for treatment.
Was walking along the crane when he slipped and fell approximately 4' to the ground straining his lower back.
Individual was hit by fly rock while setting off a shot.
