Fall to the walkway or working surface

It had been freezing rain over night and in the morning. The employee was headed to the facilities from the scale house. EE stepped off of the steps to the ground and EE's feet came out from under EE and EE fell, breaking EE's ankle.

Was opening the doors on a Conex Box and the doors bound up. Employee was trying to get the doors open and right shoulder popped.

Was loading a screen into the loader bucket, was not wearing gloves and got finger caught on the edge of the screen cutting right index finger.

Was walking to a skid loader when ee tripped and stumbled, tried to catch self and fell into the spoon attachment on the skid loader.

Was working on a scalping screen removing a bolt with right hand. Employee turned with shoulders to hold the bolt with the left hand when their back popped.

Struck by falling object

EE was helping a co-worker remove a guard from Q-173 portable crusher and the guard slipped and fell hitting EE's right shoulder.

Was kneeling on gravel working on a conveyor and been kneeling for an extended period of time and when EE went to get up experienced sharp pain in the shins.

EE went to use the facilities and when EE returned EE was going to clean off shoes with the mud jack, when EE lifted foot the other one slipped out from under EE causing EE to fall on back hurting left hip, right knee, left shoulder and head.

Was closing the passenger side door to the haul truck EE was about the enter and EE closed the passenger side door on EE's right thumb crushing EE's thumb.

Was servicing piece of equipment when the radiator door strut on the skid loader failed and came down on left forearm, with some velocity.

Was helping to dislodge a rock wedged in the tailgate of a dump truck. When the rock fell out the tailgated slammed shut on the second finger of the left hand cutting the finger.

Employee was completing his pre-shift equipment inspection. after clearing ice from the windshield he attempted to enter the cab of his loader and slipped off a rung on the ladder. He fell approximately 80 inches to the ground.

EE WAS SERVICING A HAUL TRUCK. HE ATTEMPTED TO GREASE FITTING. FITTING WOULD NOT TAKE GREASE SO HE TRIED TO EXERT PRESSURE ON GREASE GUN. AFTER SERVICING THE TRUCK HE FELT PAIN IN HIS LEFT SHO ULDER. PER DR. INSTRUCTIONS, LIMIT LIFTING TO 30LBS.
