EMPLOYEE WAS USING A 5' PRY BAR TO SHUT THE BACK DOOR OF THE CONE WHEN IT SLIPPED AND STRUCK EMPLOYEE IN THE FACE RESULTING IN 5 STITCHES.
Employee was repairing a stacking conveyor when EE got debris in left eye from blowing sand and dust which resulted in a swollen left eye at 10:00 pm on the date of incident. Swollen eye was diagnosed as corneal ulcer/small abrasion.
Employee had scratched cornea from fine material in right eye due to windy conditions.
Hearing test report indicates a recordable standard threshold shift. Miner declined the opportunity to retest.
EE was climbing a ladder to access a screen platform and exasperated a preexisting back injury. EE was treated at a chiropractor and did not lose time.
EE was cutting a hole in a piece of rubber belting using a folding utility knife. The blade dislodged from the knife. The blade flipped back and lacerated the pinky finger on the right hand. The laceration required three sutures to close.
EE was removing a bolt using a 1/2" electric impact wrench. EE was holding the bolt head with left thumb and index finger. The bolt head spun and lacerated ee's left thumb which required three sutures.
Employee was lifting a piece of rubber belting 24" x 72" up onto a screen deck and injured their back.
The worker was dislodging clay stuck in the cone crusher using a bar. The worker hit hand on the guard and fractured the pinky finger on left hand.
EE was setting up a stacker. EE's left thumb got caught in between the base and the kingpin assembly after EE loosened the support bracket used for transport. EE's left thumb was fractured and required surgery.
A welder suffered from symptoms of heat exhaustion and was brought in for medical treatment and evaluation.
EE was replacing edge segments on the loader bucket. EE tried to hold the attachment bolt in its seat and it spun lacerating the palm on his left hand. EE received stitches to close the laceration and returned to work.

EE was lifting rocks out of a jaw crusher and strained his back. Worker treated at his chiropractor. Crusher did not work on 12/3/13 and 12/4/13 because of a snow storm.
EE was driving a Cat D350E Series II Articulated Dump Truck and hit a bump that caused him to strike his rib cage on the right side arm rest causing contusions and fractured ribs on his right side.
EE claims neck pain after operating a loader. Ulland Bros was notified of work injury on 10/21/13.
Employee was tightening the hold-down bolts on the lower screen deck of a Cedar Rapids 6x20 Screen. Employee suffered a lumbar strain/spasm while getting up from a crouched position. Employee was treated at the Virginia Regional Medical Center and prescribed Ibuprofen and Flexeril. He may return to work without restriction as limited by pain.
EE stepped on a rock and sprained his left ankle.
EE dropped a guard on to his right ring finger causing a laceration that required 5 stitches.
Miner was removing a pin from a loader tooth on a 980G loader. She was using a 5 pound hammer and a chisel. The miner missed the chisel/pin and hit her left thumb causing a fracture and laceration.

EE was standing up two steps on a ladder and pushing down an a 24" adjustable wrench. The EE dislocated his right shoulder while applying downward pressure to the wrench.
EE was removing a guard from #2079 Cedar Rapids Srcreen and got his left hand pinched between the guard and the hand rail.
EE's left little finger was fractured when a 10" rock rolled down on his finger while he was clearing a rock jam in the crusher jaw. X-rays on the date of occurrence did not reveal the fracture.
Worker was cleaning out a feeder bin using a pry bar when a 4" rock rolled dowm and hit the top of his left hand causing a contusion and possible slight fracture.
Employee jumped off a conveyor elevated 2 feet off the ground. Employee twisted/sprained his left ankle and fell. Employee has a left ankle sprain and a left knee contusion.
Slabs of concrete were caught in the Jaw. The worker was using a pry bar in an attempt to dislodge the concrete slabs. The worker's left hand was caught between the pry bar and the guardrail around the jaw.
Miner was assisting in moving a scale house. A beam fell out of the loader bucket and bruised miner's lower left leg.
EE was pounding in a wedge with a sledge hammer and the hammer struck the tip of his left index finger causing a fracture and laceration
EE was using a pry bar to perform crusher maintenance and strained his back. Initial injury was further aggravated while operating the loader.

EE WAS CUTTING OFF A BEARING USING A CUTTING TORCH. A PIECE OF SLAG FELL ON THE HOSES AND BURNED A HOLE. THE EE ATTEMPTED TO KNOCK THE SLAG OFF THE HOSES AND THEY FLASHED CAUSING A BURN TO HIS RT. HAND.
WORKER CAUGHT HIS FINGER BETWEEN AN AXLE AND WHEEL WHILE REMOVING THE WHEEL FROM A BOBCAT SKID.
A CAT 330B BACKHOE WAS BEING USED TO DREDGE SAND FROM A POND. THE GROUND SUPPORTING THE BACKHOE COLLAPSED CAUSING THE BACKHOE TO TIP INTO THE WATER. THE OPERATOR EXITED THROUGH A WINDOW OPENIN G. THE OPERATOR INCURRED A LACARATION TO HIS LEFT FOREARM. THE LACERATION WAS CLOSED WITH STITCHES AT THE ALBERT LEA MEDICAL CENTER.
EMPLOYEE ATTEMPTED TO DISLODGE A PIECE OF CONCRETE CAUGHT IN THE JAW OF THE CRUSHER. THE EE'S RIGHT HAND GOT PINCHED BETWEEN TWO PIECES OF CONCRETE.
EE WAS ATTEMPTING TO ADJUST FEED GATE OPENING BY SHORTENING THE 3/8 INCH CHAIN THAT HOLDS THE GATE OPEN ON THE FEED CONVEYOR UNIT 2200 THAT FEEDS THE CRUSHER. HE OPENED THE GATE & SECURED IT W ITH A BLOCK. THE BLOCK SLIPPED WHILE HE WAS ADJUSTING THE GATE & PULLED HIS RIGHT INDEX FINGER & THUMB THROUGH THE CHAIN OPENING CAUSING LACERATIONS.
WORKER WAS LIFTING WATER CONTAINERS OUT OF A CAR TRUNK & EXPERIENCED BACK PAIN.
SHOVELING UNDER CONVEYER AND TWISTED BACK.
EE AND COWORKER WER LIFTING A STARTER MOTOR INTO A 990 LOADER. EE HURT LEFT SHOULDER DURING THE LIFT.
EE'S RIGHT INDEX FINGER WAS INJURED ON 5-1-02 AFTER IT WAS HIT BY A HAMMER WHILE HE WAS HOLDING A WRENCH. EE WENT TO THE CLINIC ON 5-24-02 ANDIT WAS DETERMINED THAT HIS FINGER IS FRACTURED.
