EMPLOYEE WAS USING A 5' PRY BAR TO SHUT THE BACK DOOR OF THE CONE WHEN IT SLIPPED AND STRUCK EMPLOYEE IN THE FACE RESULTING IN 5 STITCHES.

Struck by flying object

EE was cutting a hole in a piece of rubber belting using a folding utility knife. The blade dislodged from the knife. The blade flipped back and lacerated the pinky finger on the right hand. The laceration required three sutures to close.

EE was replacing edge segments on the loader bucket. EE tried to hold the attachment bolt in its seat and it spun lacerating the palm on his left hand. EE received stitches to close the laceration and returned to work.

EE was attempting to straighten a lip on a conveyor/hopper for the overhead feed belt on a Fabtec closed circuit cone and screen plant. EE was standing up two steps on a ladder and pushing down an a 24"" adjustable wrench. The EE dislocated his right shoulder while applying downward pressure to the wrench.

EMPLOYEE SEPEARATED CORTAL CARTILAGE IN RIGHT RIB CAGE WHILE LYING ON HIS STOMACH TO RACHET A BOLT LOOSE TO CHANGE THE LIP RUBBER ON THE CRUSHER. HE DID NOT HAVE ANY LOST TIME, BUT HE HAS A 5 LB LIFTING RESTRICTION.
