Employee was walking down stairs, He began to fall and reached for hand rail. His falling weight caused him to strain his right triceps.

Employee fell approx 18 inches when he lost his balance when climbing on to equipment. He hit the back of his head on the ground causing a contusion.

Employee was walking on a pile of 4 inch stone when he slipped and twisted his right knee. Injury was not reported to the Company until 07/19/2012. He was diagnosed with a meniscus tear but elected to post pone surgery until winter shut down. His first day of missed time was 1/15/13.

Employee was cutting a piece of steel in half with a torch. The cut piece fell and hit employee's left knee & thigh causing a contusion.

Employee was tightening a "J" hook during a screen change using a wrench when he strained his left shoulder.

Employee complained of right wrist pain from operating joy stick on 992 loader, on 2/26/12. Diagnosis 5/18/12, Operation 6/6/12. No lost time or restrictions until 6/6/12.

Employee was sliding an electric motor from a loader bucket onto a truck bed when he pinched his right index finger between the motor and the truck bed causing a laceration.

EE was striking a wedge with a sledge hammer. He missed the wedge and the hand holding the hammer handle struck against the wedge causing a contusion to right index finger and nail.

Employee was helping remove a crusher when he tripped & fell resulting in a strain to lower back.

Employee was shovelling stone when he strained his lower back.

Employee stated that he was climbing the ladder to get into 988 Loader cab when he felt pain in lower back.

While climbing ladder to get into a front-end loader, employee slipped and scrapped his lower leg. On 7/21 he indicated leg was swollen and was sent to medical clinic for treatment diagnosis: cellulites

Employee was removing upper engine panel on 980 Loader. After removing 3 of the 4 bolts, the panel swung down, striking the employee's left forearm causing laceration requiring 9 stitches

Employee was adjusting a conveyor belt, his wrench hit the conveyor guard, dislodging it from its hook. The guard fell, hitting the employee finger causing a contusion.

Employee was removing a pump drive & hydraulic pump from a drill rig when it fell on his right foot causing a fracture to right 2nd toe & contusion to right foot.

While another employee was lowering a screen down it slipped and hit employee on left foot causing a contusion.

Employee received an insect bite on left fore arm. After several days, the arm became swollen and infected.

Employee stated that while shaking out dust from a rag after wiping down truck dashboard, the tip of rag hit him in his right eye causing abrasion.

Employee was cleaning dirt build-up around a head pulley using a metal bar. The bar got caught in the pulley, causing the employees thumb to get pinched by pulley head.

Employee was removing a hose clamp from a water pump when he felt a pop in his right ring finger. Diagnosis: Tendon strain.

Employee stated as he turned to sit down in his CAT excavator he felt a pain in his lower back - right side

Employee was hit in the right ear by an air gun that flew out of a co-workers grasp causing laceration behind right ear requiring 5 stitches.

Employee was kicking a breaker bar to loosen a drain plug when he slipped. Employee stated he was still feeling pain in October and was sent for an MRI on 10/8/2009 which revealed an ACL Tear.

Employee and Co-worker were handing up a 95lb. screen to another emplyee. He indicated afterward that he felt pain in left shoulder.

Employee was putting air in low tire on pick up truck. The tire exploded sending grit into employee's right eye causing abrasion.

Employee was striking an allen key socket into an allen key bolt with a hammer. A piece of the socket broke off and hit him on his left forearm causing laceration - 6 stitches.

Employee stated he had lower back pain he had a previous work related back injury in February 2008.

Employee was checking for a leak from a drill rig hydraulic hose. As he moved hoses around, pressurized fluid shot out & caused a puncture wound to his left middle finger tip.

Employee was climbing out of a chute under #4 shaker when he felt a "Pop" in his right knee. Diagnosis: Right Knee Strain - possible Medial Meniscal Tear.

Employee was arc welding in a chute. That night he experienced a burning sensation in his eyes. He went to his local emergency room and was diagnosed for flash burn.

Employee was cleaning a shaker chute with a 20 pound chipping hammer, when the hammer broke thru encrusted material, he was pitched foward and felt lower back pain.

Employee was climbing down the counter weight ladder of a front end loader. He missed the last step and fell to the ground causing a bruised sacrum.

Employee was using a utility knife to cut a plastic bottle. The knife slipped and cut employee's right wrist - 3 stitches plus tetanus shot.

Employee was shoveling stone, the shovel hit something solid causing sprain to right wrist & forearm.

Employee was removing bolts on a surge tank cover using an air impact gun. When a bolt broke, the gun fell out of his right hand and struck his left index finger, causing a laceration and contusion.

Employee was sitting in haul truck, when he closed the truck door, his left thumb got caught between the door and the door frame. This caused a laceration, requiring 4 stitches.

Employee was sitting on a bench in locker room, when he bent over to put on his work boots, he felt a pain in his lower back.

Employee was cleaning area around a feed plate using a steel bar. The bar slipped causing employee'e right hand to strike a dust suppressor fitting causing a fracture to his right ring finger.

Employee was removing a broken feeder plate and after the last bolt was removed, the plate released, striking employee on the back of his hard hat.

Employee climbed over a "Keep Out" barrier on crusher and fell approx 17 feet thru a hole in decking. Causing concussion, sprain, fracture & contusion.

EE was using a wire wheel grinder. He dropped the tool and the spinning wire brush struck him on his left thigh, causing an abrasion to his skin.

EE states that he had pain on his left elbow for some time from shifting gears in front end loader.

Employee was holding a metal pipe under a rock hammer as it was being lowered into a box for shipping. As the hammer fell into box, the pipe was jerked downward causing back pain to employee.

Employee stated he developed pain in his left elbow from operating equipment over time.

Employee was carrying a 4' x 8' shaker screen with a co-worker when he felt pain in his lower back.

Employee stated that when welding with MAP gas, he inadvertantly touched his shirt. He tried to put out flame with his hand resulting in burn to chest & hand.

Employee was freeing a jammed stone feeder with a pole when a 6 inch core stone was dislodged and struck the top of the employee's left hand causing an abrasion / contusion.

While changing screens, employee reached under screen to retrieve a bolt while screen was being lowered. Screen edge hit employee on shoulder& back of neck causing bruise / contusion.
