Employee was walking on a pile of 4 inch stone when he slipped and twisted his right knee. Injury was not reported to the Company until 07/19/2012. He was diagnosed with a meniscus tear but elected to post pone surgery until winter shut down. His first day of missed time was 1/15/13.

Employee was sliding an electric motor from a loader bucket onto a truck bed when he pinched his right index finger between the motor and the truck bed causing a laceration.
EE was striking a wedge with a sledge hammer. He missed the wedge and the hand holding the hammer handle struck against the wedge causing a contusion to right index finger and nail.
