Employee was arc welding in a chute. That night he experienced a burning sensation in his eyes. He went to his local emergency room and was diagnosed for flash burn.

Employee was removing bolts on a surge tank cover using an air impact gun. When a bolt broke, the gun fell out of his right hand and struck his left index finger, causing a laceration and contusion.
