Caught in, under or between a moving and a stationary object

Employee was sitting in haul truck, when he closed the truck door, his left thumb got caught between the door and the door frame. This caused a laceration, requiring 4 stitches.

Fall from machine

Employee climbed over a ""Keep Out"" barrier on crusher and fell approx 17 feet thru a hole in decking. Causing concussion, sprain, fracture & contusion.

An employee using a sledge hammer to tigten a wedge plate bolt struck fellow employee who was holding slugging wrench on left bicep when the hammer glanced off the wrench.

Employee was climbing a ladder of a 988 loader for Am Start-up. His hand slipped off the hand rail causing him to fall backwards, landing on his back. he received a lower back strain,

Employee was cutting nuts with a torch to release bolts on a 980 loader cutting edge. As the nut was split, pressure on the nut caused it to release and it struck the worker in left knee.

EMPLOYEE WAS PERFORMING ADJUSTMENT ON STONE CRUSHER WITH A WRENCH WHICH WAS BEING STRUCK BY A CO-WORKER USING A HAMMER. EMPLOYEE'S FINGER GOT PINCHED BETWEEN WRENCH HANDLE AND CRUSHER FRAME.
