The drivers were told by the site foreman to dump anywhere deemed safe. When the driver went for the first dump, EE backed against berm and attempted to dump. It was slightly uneven ground and the dump gate locked so they tipped over. Had to be helped through windshield. Dump truck had to be towed away. Ambulance was called. EE sustained cut on head and bruised arm.

Stated the condition from the previous injury had returned, Old insurance company reported that the employee stated they picked up a bag and felt something in the same location of the previous injury.

Dr. claims injury could be from repetitive motion of ee's job. Not from a specific incident. Company disputes this claim and will seek further evidence of work related cause.

EE was stepping out of Truck #28 at stockpile area to get in the loader. As he stepped down from the truck his left foot came down on a rock and his ankle twisted causing an avulsion fracture. EE declined medical treatment at time of incident then on 3/17/14 decided to be seen.

Employee was working on Door400, the 400 ton hopper Mega door. While working on the door, the fabric lifting strap that raises and lowers the door broke and the door fell 8-10' down and struck employee on the head knocking his hard hat off. The door then struck his left shoulder and forearm that was on the railing of the scissor lift.

Employee passed out at work. Upon questioning & evaluation by shift foreman, employee attributed passing out to his diabetes. Employee refused medical treatment and went home at the end of his shift. From home he went to the doctors/hospital where it was determined he had a heart attack.

Employee was working on classifier 230B trying to move a bound up part when his hand slipped off and collided with the classifier housing in an area that was worn to a sharp edge. Employees right hand grazed the sharp edge causing a laceration at the base of his right thumb that required sutures.

On 10/24/12 EE strained his rgt. side back and hip area while installing a water line. EE declined medical attention at that time. EE reports that on 11/28/12 he further aggravated the injury while performing the same task. He worked through the day. Worked 11/29/12 then sought medical treatment that evening in the emergency room and was placed out of work.

EE was directed to weed whack around the NYCO grounds. He reportedly came into contact with poison ivy. By 7/9/10, EE experienced a severe allergic reaction to the poison ivy and sought medical treatment.

In doing so some ice fell out of the bucket onto the floor. EE then stepped on the ice and his feet went out from under him. he landed full weight on his shoulder.
