Temporary employee got hand in-between the rollers on a roller mill and injured hand.

Struck against a moving object

The drivers were told by the site foreman to dump anywhere deemed safe. When the driver went for the first dump, EE backed against berm and attempted to dump. It was slightly uneven ground and the dump gate locked so they tipped over. Had to be helped through windshield. Dump truck had to be towed away. Ambulance was called. EE sustained cut on head and bruised arm.

Trying to close trailer gate - hinge portion would not close - pinched finger in gate.

Fall to the walkway or working surface

EE conducting sampling in pit at Oak Hill Mine (uneven work surface with loose material), slipped rolled right ankle & fell on lft hip. Appears to be a hematoma to lft hip on 7/21/16 received diagnosis of hairline fracture to rgt. ankle.

While dismounting excavator employee slipped on track and fell hitting right hip on track of excavator then landed on ground hitting left hip.
