EE was cutting a zip tie off of the portable screening plant. EE hit the back of their hand on a guard and cut self.
Employee wasn't paying attention to where EE was going and walked into a guard with their face. EE has damage to the teeth and they're checking EE's head and shoulder for other injuries.
Wood got stuck in the tail pulley. Employee used a piece of pipe to hit it out. Swung pipe, Clipped their finger and tore the fingernail.
While sharpening drill bit the grinding tool disengaged and hit the operator in the ankle causing a contusion.
Moving rocks out of the jaw, a rock slipped and jammed EE's right index finger.
Employee was cleaning the windows on loader and slipped and fell from the machine.
The employee was climbing onto a loader to begin shift. A knee banged into the second step.
Excavator operator opened the door to the cab. Operator was unable to close it, due to the way the machine was facing. Operator decided to spin the machine and the door slammed shut on left elbow.
While removing screen from screening plant, screen dropped on EE's hand.
while using an electric disc grinder, EE got a piece of debris under EE's safety glasses into EE's left eye
Employee was moving rocks by hand, wearing gloves, and right middle finger got pinched between two rocks.
Employee was loosening the ball valve on the Deister Triple Deck Wash Plant. Employee was using a pipe wrench and felt a pain in this shoulder. Employee left work and consulted a Doctor.
While the employee was adding hydraulic oil to their machine they slipped and fell.
Employee was driving a haul truck, hit a bump and hit their head on the roof. Employee said they were wearing the seatbelt. Driver had the seat adjusted too high, was possibly driving too fast and wasn't reading the road. Employee did not report the incident until 2/16/17 after a Dr's visit where they were given the diagnosis of cervical strain.
Worker walked into a safety screen while checking a problem with the portable plant. Worker was wearing a hard hat and safety glasses and still scraped upper cheekbone, nose and eye.
Employee was working on a stalled conveyor. There was a small amount of loose gravel on the belt and employee slipped and fell injuring back.
Loader operator was climbing into his machine and he felt a tear in his right calf. He went to the doctor's on Saturday, 9/19/2015 and it was determined that he ruptured his right calf muscle.
Employee was lifting himself into the crane and hurt his left shoulder. Doctor believes that it is just inflamed/strained. Employee has been prescribed six weeks of physical therapy. He was told he can return to work on Monday, 9/21/2015.
Slip and Fall due to extremely low temperatures and icy conditions in the yard. Sand had been put down to prevent this, but the sand and snow wore down and this icy patch unknowingly was exposed as workers traversed the area.
Rocks bridged up in the hopper and spilled over the edge. When EE went to clear the jam - he rolled his ankle on the spilt rocks while walking to the hopper.
Youth on motorcycle was fleeing from the police. His bike had no lights and he was not wearing a helmet. He left the main road (rte 130) and turned onto richs way. Its unclear what happened next but a police officer found the bike under our gate (locked) and the youth unconscious on the ground.
Arm came in contact with 110 electrical panel. Elbow touched ungrounded light switch resulting i electric shock to right side of body resulting in no damage or impairment.
Employee was on the sand screw putting on rubber, it was lightly raining. EE slipped & hit his face & then landed on his right knee & hand. He broke 2 bones in his hand.
Employee had come down off the plant and onto the ground when he slipped on ice and fell onto his back. He got up and thought he was fine until he woke up the next morning and his back was sore.
EE SLID OPEN THE WINDOW OF THE OFFICE TRAILER. THE PRESSURE OF EE PALM AGAINST THE GLASS CAUSED THE WINDOW TO SHATTER. EE ARM WAS CUT AND EE DROVE SELF TO THE HOSPITAL, EE WAS GIVEN A TETNUS SHOT AND XRAYS WERE TAKEN, EE ARM WAS BANDAGED AND CLEARED TO RETURN TO WORK NEXT DAY. AS OF 03/11/05, EE HAS NOT RETURNED TO WORK, GOING TO HOSPITAL, EE IN PAIN, POSSIBLE INFECTION
EE LACERATED HIS LEFT HAND CHANGING STEEL IN CRUSHER. HE WAS WEARING GLOVES BUT THE DUST RING WAS SO SHARP IT CUT RIGHT THROUGH HIS GLOVE. HE WAS TREATED STITCHES AND CLEARED TO RETURN TO WORK .
