Fall to the walkway or working surface

Slip and fall on ice on the catwalk. Torn meniscus in one knee.

Struck against stationary object

EE was cutting a zip tie off of the portable screening plant. EE hit the back of their hand on a guard and cut self.

Caught in, under or between a moving and a stationary object

Excavator operator opened the door to the cab. Operator was unable to close it, due to the way the machine was facing. Operator decided to spin the machine and the door slammed shut on left elbow.

Arm came in contact with 110 electrical panel. Elbow touched ungrounded light switch resulting i electric shock to right side of body resulting in no damage or impairment.

Ee was watch the plant when he heard a funny noise. He stepped up on the first deck to look in the crusher. He stepped back and turned to the left. His left knee popped and he collapsed on the deck in pain. He called other & 911 was called immediately. He was torn the tendons in his left knee earlier this year he had surgery on his right knee-non work related injury.

A PIECE OF STEEL GOT STUCK IN THE CONE CRUSHER EE AND OTHER WERE USING A LONG BURNING ROD TO BURN THE STEEL OUT OF THE CRUSHER. ONCE THE STEEL WAS REMOVED HE PLACED THE BURNING ROD INTO A BUCK ET OF WATER TO COAL. THE WATER SPLASHED INTO EE'S FACE AND HE THINKS THAT S WHEN THE DEBRIS ENTERED HIS EYE.

EE WAS REPLACING THE HOSE ON THE EXCAVATOR. HE HAD TO MOVE WIRE OUT OF THE WAY (HE TOOK OFF HIS GLOVE FOR BETTER MEASUREABILITY). HE CUT HIS THUMB WITH A UTILITY KNIFE.
