Showing all 4Employee was standing on excavator steps while fueling excavator. One foot slipped from excavator step; employee pitched forward and stopped fall with one hand by placing same on excavator deck. The impact stress of stopping his fall resulted in rotator cuff injury.
Employees were fabricating stacker/ conveyer sections. EE was uncoupling attachment from stacker eye. EE slipped.
EE climbed on top of truck to assist with placement of pipe in dump bed. Wind speed was around 14 mph. Surface not intended working surface. He fell off truck.
