There were 3 raccoons sleeping in a hopper, not visible as the hopper is 15ft +/- above the ground. When the machine started, 2 of the raccoons were crushed, however, the 3rd jumped out of the hopper and went down EE's arm, where it injured EE's left hand.

EE was working in a loader and was stung by a bee, which EE is allergic to.

Slipped on steps of 980M loader

Employee was greasing fuel tank and jerked grease line and handle came out and smacked Employee in the face/eye.

Slipped and fell on track while climbing off CAT 374 Trackhoe. Scraped skin on right shin.

Struck against a moving object

EE was asked to stockpile rock at stockpile area. He was backing up across pad and struck an F-250 in front left corner. The F-250 was parked with nobody in it at time of accident. At this time I was notified and went to the scene of the accident. EE was sent for a drug screen at 10am, the very next morning (no personal injuries occurred from this accident)

Fall onto or against objects

Employee was standing on excavator steps while fueling excavator. One foot slipped from excavator step; employee pitched forward and stopped fall with one hand by placing same on excavator deck. The impact stress of stopping his fall resulted in rotator cuff injury.

EE climbed on top of truck to assist with placement of pipe in dump bed. Wind speed was around 14 mph. Surface not intended working surface. He fell off truck.
