Operator was changing the bottom deck top screen in our KPI screen as EE was climbing out from under the rubber flap on the rear of the screen the worn screen tipped over which struck EE on the back left portion of EE's head which caused EE to strike EE's right side of EE's head against one of the protruding bolts on the rear of the screen

Operator was cutting a conveyor belt section. EE was almost all the way through the belt when the knife slipped through and hit EE's upper leg. About 8" up from the knee.
Climbing on the shot, hooking up Detonators, felt a pop in right knee.
EE was adjusting a tail pulley with a breaker bar. EE was using a wrench to loosen the pulley when the breaker bar loosened suddenly, causing EE's body to jerk and fall to ground landing on right knee. Injury resulted in an indicated meniscus and ACL tear.
Employee was using a bar to poke chute when a rock fell onto the bar. Employee held on to bar as it pulled him, causing a muscle strain in his left shoulder
Employee was attempting to hook up trailer to the back of a service truck. While pulling on the tongue of the trailer, he felt a pull in his lower back. He was taken to Industrial Medical Associates to be looked at. Light duty per doctor's request.
While performing morning pre-shift inspection, employee opened haul truck engine compartment hood when he felt something pull in his shoulder. Originally treated with first aid. Later advised to get an MRI, which showed a rotator cuff injury.
Employee was changing the cutting edge on 952 Kawasaki loader with an air wrench and socket. Employee checked the bottom of a bolt with his hand to see if it was spinning. The bolt was spinning and had a sharp edge which cut through his glove and lacerated his right index finger requiring seven stitches.
Rainy day employee was working at the primary crusher when he slipped and struck his right knee on the flywheel sheave. First aid only for one week and was sent to the doctor on 6/13 for further treatment. The Doctor placed employee on restricted duty starting 6/14. Employee advised Plant Manager on 6/7/13 regarding his knee injury.
Employee was performing maintenance work on the jaw crusher. While installing the stationary jaw and wedge it suddenly shifted and struck employees left leg.
The crew at the mine were performing some crusher repair maintenance, and were required to conduct some cutting and welding. There must have been some smoldering slag that was not noticed by the crew and the belt on 1C conveyor caught fire. The fire was put out by the local fire department and the damage was approximately $7000.00 and one lost week of production.
Trying to break a coupling loose from the steel on a IR 590 drill, the steel slid and hit the centralizer causing a piece of steel to break off and stick in his arm.
Employee was installing a dump body lift cylinder on a 773 haul truck. When the support rigging shifted the cylinder fell against the wheel hub of the truck. The employee's wrist was caught between the cylinder and hub, resulting in a broken wrist.
Cutting rubber tabs off of a screen panel. Knife slipped and cut left index finger. 3 stitches were required. Returned to work the same day.
Using pry bar and porter power to repair water drain for belt press. Pry bar slipped and jarred his left arm. Neck Sprain.
2/20/09 (Friday) was welding and turning I-beam. Reported pain in his elbow to supervisor on Monday 2/23/09. Employee was then sent to medical provider who put him on light duty and anti-inflammatory.
Working on the N quarry road (pushing mud with dozer). Rubbed his eye with hand and got something in his eye. Employee attempted (unsuccessfully) to flush the object out with wash and with a Q-tip. Foreign body in eye.
Employee was changing air line under the bin. When the air line was removed, air blew in employees face and stone dust blew into left eye.
While running a loader at the muck pile, movement of the equipment allegedly caused muscle strain in neck. DX: Trapezius Pain
While using a utility knife, the knife slipped off and he cut his leg. Stitches.
Experienced lower back pain while carrying a 20lb. Propane tank from the pole barn. Sent to Provider for low back strain. MD Ordered: Physical Therapy & Prescription medication (anti inflammatory).
While changing screen on plant 32, he struck his left elbow on side of equipment. The next day elbow was tender and greatly swollen. Sent to provider, given prescription anti-inflamatory medication to reduce swelling and prevent infection.
While hosing tail of jaw crusher, pressure of hose forced him backwards, bruising and twisting rt. knee. given meds and pt.
Employee was using a hoe to pull rock off of shot. Another rock rolled off screen and struck ee in month. We recently received aother and dx from dentist.
While stepping off the 3' ladder employee was using, ladder kicked out causing him to fall, injuring right ankle. Employee is currently on light duty program, no lost time, physicial therapy.
Employee of Orica initiated a successful blast. The NYS Police called Callanan's to report a bus had been hit on the NYS Thruway. MSHA was notified. It was determined that a rock pierced the roof of the W bound bus. 3 people were injured: #1 in the neck & back, #2 in the leg. #3 treated for abrasions. Victims transported to St Mary's Hospital in Amsterdam and released.
While installing guard on HP feeder belt, ladder that employee was standing on became off balance due to stone underneath ladder. Ladder kicked out & employee's left foot caught in ladder. He fell, injuring left ankle. He's currently on light duty. No lost time.
Checking on material in a bin & at crushing plant, stepped on a rock, twisting left ankle.
Grinding steel in close quarters, a piece of material went into his eye. Antibiotic eye solution on 5/17/06.
While using a blow torch to cut screen feed of trougher, oxygen line on torch suddenly blew causing flash back burning left forearm.
Installing wedges that hold ball liner in crusher. Was holding wedge in one hand and using hammer in other hand. Struck tip of right thumb with hammer.
While replacing screen cloth in mill, he turned to get a screen when he knocked his hard hat off. When attempting to retrieve hat, he struck his forehead on spray bar - incurred laceration of forehead - 3 sutures.
WHILE CLOSING WATER VALVE @ BIN SITE, SLIPPED & FELL ON ICE, FALLING & INJURING HIS RT. ARM & SHOULDER. SEEN IN E.R. & AN X-RAY WAS TAKEN TO CHECK FOR ANY FRACTURES. CONTINUED TO BOTHER HIM, REFERRED TO ORTHOPEDIC M.D. THE FOLLOWING WEEK & LATER STARTED P. THERAPY.
Walking in crushing area and replacing drive belts on crusher, stepped on rock, twisting left knee. Saw provider 6 days later when it began bothering him. Sent now to physical therapy for a few weeks, beginning on 11/7/05.
EE WAS CLOSING DOOR ON BACKHOE WHEN SUDDENLY THE HANDLE OF BACKHOE BROKE OFF, CAUSING LACERATION OF LEFT THUMB. SENT TO ER AND RECEIVED 9 SUTURES.
WHILE CHANGING SCREEN, HAMMER DROPPED INTO BIN. EE REACHED IN TO RETRIEVE IT, INCURRING PUNCTURE WOUND ON RIGHT THUMB FROM JAGGED ROLL BAR. DEVELOPED INFECTION - PLACED ON ANTIBIOTICS.
WHILE OPERATING LOADER. GLANCED OVER HIS LEFT SHOULDER TO MAKE SURE HE WAS CLEAR. THIS STRAINED HIS NECK TO THE POINT HE COULD NOT STRAIGHTEN WITHOUT PAIN. WAS SEEN BY Occ Medical Services. X-RAY NEGATIVE DX: CERVICAL STRAIN AND ACUTE MUSCLE SPASM. WAS STARTED ON PHYSICAL THERAPY & PLACED ON LIGHT DUTY.
Employee was working in the dredging area, as he was climbing off of the dragline his pant leg caught the handle of a shovel which was leaning against the equipment. The handle lacerated his groin area, causing a testicular tear. Transported by ambulance to the E.R., the wound was surgically repaired.
While carrying 5 lb rubber down stairs on tower #2, he missed the bottom step and fell to the ground, striking his right shoulder.
In 1980 he had an injury which resulted in an umbilcal hernia. Employee has continued to work for the past 25 years with untreated hernia but decided to have surgery this year. Reported it to supervisor on 1/27/05 and had surgery resulting in hernia repair; no lost time.
EE WAS STANDING ON SIDE OF SCAFFOLD, ABOUT 1.5' FROM GROUND, REMOVING ECCENTRIC FROM CRUSHER. EE'S FOOT SLIPPED AND HE FELL TO THE GROUND, STRIKING HIS BACK (CERVICAL & UPPER BACK CONTUSION & STRAIN). EE WAS SEEN AT EMERGENCY ROOM AND RELEASED TO FULL DUTY. CONTINUED TO BOTHER HIM, SO EE WAS STARTED ON PHYSICAL THERAPY FOR A FEW SESSIONS. CONTINUED TO DO REG. DUTIES.
WELDING OVER HEAD ON CONVEYOR WHEN SLAG DROPPED DOWN IN FRONT OF SHIRT. DID NOT THINK ANYTHING OF IT, UNTIL 1/4/05 WHEN HE DEVELOPED INFECTION FROM SLIGHT 2ND DEGREE BURN ON ABDOMEN. PLACED ON ANTIBIOTICS.
SHOVELING WET ROCK DUST WHEN HE HAD A PAIN IN HIS LOWER BACK. WAS TAKEN TO ER, TREATED AND DISCHARGED. NOW GOING FOR PHYSICAL THERAPY BUT WORKING REGULAR DUTY.
CLEANING OIL, MUD IN CHUTE IN TOWER #1. A CHUNK OF MUD FROM BUILD-UP IN SCREEN FELL ONTO BACK OF NECK. GOING FOR PHYSICAL THERAPY 2-3 WEEKS. DIAGNOSIS: NECK STRAIN AND CONTUSION.
Using 30" pry bar to knock mud from return rollers. Bar caught under roller, bounced out striking employee in cheek.
PILE OF MATERIAL HAD BEEN LEFT ON ROAD. EE WAS DRIVING VOLVO OVER THE HAUL ROAD. EE DROVE OVER THE PILE AND WAS JOLTED AGAINST A PIECE OF METAL ON THE TRUCK. LACERATED RIGHT EAR - PLACED ON ANTIBIOTICS/STERI-STRIPS.
While moving steel, picked up piece with sharp edge, which lacerated his finger. taken to occ health clinic 7 sutures
Putting platform on drill rig, when wrench he was using slipped off. Left arm slipped and he felt pain in left shoulder. Shoulder strain.
EMPLOYEE HAD OIL ON SOLE OF BOOT AFTER CHANGING OIL IN GEAR BOX OF DIESTER SCREEN. STEPPED ON STEEL STEP OF CAT WALK, SLIPPED & FRACTURED LEFT HEEL. PLACED ON LIGHT DUTY.
