Experienced lower back pain while carrying a 20lb. Propane tank from the pole barn. Sent to Provider for low back strain. MD Ordered: Physical Therapy & Prescription medication (anti inflammatory).

Employee was working in the dredging area, as he was climbing off of the dragline his pant leg caught the handle of a shovel which was leaning against the equipment. The handle lacerated his groin area, causing a testicular tear. Transported by ambulance to the E.R., the wound was surgically repaired.

EE LIT CUTTING TORCH - SET TORCH DOWN MOMENTARILY TO PULL ON HIS CUTTING SHIELD (WAS WEARING REG. SAFETY GLASSES). RUST POPPED, BLOWING RUST PARTICLES - FLEW INTO SIDE OF RIGHT EYE. FLUSHED EY E & SENT TO PROVIDER. PLACED ON ANTIBIOTIC EYE DROPS FOR PRECAUTIONS.

FIRE ERUPTED FROM UNDER FLOOR OF LOADER, SCORCHING BOTH LEGS/ARMS OF OPERATOR WHO PROCEEDED TO JUMP FROM LOADER ONTO GROUND.

OPENING UP CHAIN LINK USING HAMMER. STRUCK LT. THUMB WITH HAMMER.

REPAIRING WATER PUMP IN CONE PUMP ROOM. SLIPPED & FELL ON WET BOARD WHICH WAS USED TO STAY OUT OF THE WATER/MUD CAUSED BY THE PUMP FAILURE. STRUCK LT. FOREARM ON METAL GUARD.
