INJURED STATES HE WAS REPOSITIONING THE POWER CABLE FOR THE LUBEWAGON IN AREA 301. WHILE HE WAS PULLING ON THE CABLE HE FELT A PAIN IN HIS RIGHT SHOULDER. HE WAS SENT TO DR. ON 10-14-96. HIS P ROBLEM CONTINUED TO WORSEN. SURGERY WAS DONE TO REPAIR A TORN ROTATOR CUFF ON 1-21-97, AND LOST TIME BEGAN.

EE STATED THAT AFTER INSTALLING SEVERAL ROOF BOLTS IN AREA 301, HE WAS MOVING THE BOLTING MACHINE TO AN OUT OF THE WAY LOCATION. THE STABLIZIER BAR MASHED HIS LEFT FOOT WHEN THE MACHINE WAS MO VED OVER UNEVEN FLOOR. HE WAS NOT WATCHING HIS FOOT POSITION CLOSELY ENOUGHT.
INJURED STATES TAHT HE WAS POSITIONUING A FLIGHTON THE 1-A CLASSIFIER LOCATED IN THE SURFACE MILL BY MANUALLY ROTATING THE CLASSIFIER DRIVE MOTOR BY PULLING ON THE V-BELTS. INJURED PULLED ON T HE BELT AND GOT HIS RT RING FINGER CAUGHT BETWEEN THE BELT AND MOTOR PULLEY CAUSING AN EVULSIVE INJURY OF HIS RT RING FINGER JUST BELOW THE FINGERNAIL.
THIIS HOIST KICKED THE OWER, AN INVESTIGATION REVEALED THAT ARKING HAD OCCURRED ON THE WINDER MOTOR ARMATURE, CAUSED BY CARBON BUILD UP AND HIGH HUMIDTY. CLEANED ARMATURE, BRUSH HOLDERS AND O THER COMPONENTS. RE-INSULATED BRUSH HOLDERS AND HOIST BACK ON LINE AT 7PM 5-31-96.
INJURED STATES HE WAS HELPING HIS CREW MOVE A SLAB OF MINE RIB OFF THE TRAILING CABLE OF THE CONTINUOUS MINER, WHEN HE FELT A SHARP PAIN INHIS BACK, LEFT SHOULDER, AND LEFT ARM. NOTE THIS EE D ID NOT BEGIN LOSING TIME FROM WORK UNTIL 5-26-96
THEY REMOVED THE CLUTCH PLATE PACKETT (HOUSING) FROM THE LEFT 4160 MOTOR GEAR BOX. IT WAS PLACED ON TOP OF THE COVER PLATE FOR THE LEFT TRACK. THE CLUTCH PLATE PACKETT SLIPPED AND FELL ABOUT 1 8" STRIKING HIS RIGHT FOOT.
AT 923 AM ON 4-16-96 THE ELECTRICAL POWER WAS CUT OFF DUE TO DIFFICULTIES SUFFERED BY S.P.S. POWER CO ALL UG PERSONNEL WERE GATHERED AND ASSEMBLED AT THE SHAFT BOTTOM UNTIL POWER WAS RESTORED AT 255 PM AFTER THE VENTILATION FANS WERE RESTARTED, COMPETENT PEOPLE EXAMINED ALL WORKING AREAS OF THE MINE FOR HAZARDOUS CONDITIONS AND NONE WERE FOUND. THE UG WORK FORCE WAS THEN ASSIGNED T
WHILE HELPING A CO WORKER LOAD HIS TOOL BOX INTOA MAINTENANCE VEHICLE, EMPLOYEE FELT A SHARP PAIN IN HIS UPPER ARM AND SHOULDER. HE WAS SENT TO A DR ON 3-14-96. CONTINUED TO WORK WITH INCREASI NG DIFFICULTIES. SURGERY WAS PERFORMED ON ROTATOR CUFF AND BICEP MUSCLE ON 4-23-96.
INJURED STATES HE WAS SHOVELING LOOSE MATERIAL FROM AROUND THE #293 BELT TAIL ROLLER AND THE SHOVEL CAME INTO CONTACT WITH THE MOVING ROLLER WHICH CAUSED THE SHOVEL TO KICK BACK AND STRIKE HIS LEFT THUMB. THE RESULTING INJURY WAS A SIMPLE FRACTURE OF THE LEFT THUMB LOCATED JUST BEHIND THE THUMB NAIL.
EMPLOYEE HAD ASSISTED IN CORRECTING AN ELECTRICAL PROBLEM WITH THE MOBILE BRIDGE CONVEYOR SYSTEM. WHEN REPAIRS WERE COMPLETED HE PROCEDED TO THE AREA TRANSFORMER, UNLOCKED THE POWER. AS HE WAS ATTEMPTING TO RETURN THE BREAKER TO THE ON POSITION, THE BREAKER FLASHED CAUSING THE INJURY.
EMPLOYEE WAS WORKING IN AREA 289 OF THE UG MINE. HE WAS ATTEMPTING TO GET ELECTRICAL POWER TO THE WELDING MACHINE. THE CABLE FROM THE WELDER HAD BEEN LAID OUT. HE WENT TO THE FAN AND PICKED UP THE 480 VOLT ENERGIZED CABLE. HE RECEIVED AN ELECTRIC SHOCK FROM ONE DAMAGED PHASE LEAD.

HE TOOK A RATCHET WRENCH WITH AN ALLEN SOCKET AND ATTEMPTED TO LOOSEN THE LUGS ON THE INCOMING POWER LEADS ON TOP OF BREAKER. A FLASH OCCURRED WHEN THE ENERGIZED POWER CIRCUIT BECAME GROUNDED THROUGH HIS METAL RATCHET WRENCH AND THE FRAME OF THE CONTROL PANEL.
EE AND COWORKER WERE REMOVING A SECTION OF 12" CHANNEL FROM THE FRAMEWORK FOR THE #1 SALT BIRD. WHILE STANDING ON A PLATFORM MOUNTED ON THE FORKS OF A FORKLIFT, HE ATTACHED A METAL CLAMP AND C ABLE TO THE CHANNEL. WHEN THE CHANNEL WAS CUT LOOSE, THE PLATE CLAMP SLIPPED OFF THE CHANNEL AND THE CABLE HE WAS HOLDING CAME LOOSE. EE LOST HIS BALANCE AND FELL TO THE FLOOR BELOW.
WHILE INSPECTING A BATTERY CELL OF A BATTERY THAT WAS BEING CHARGED, INJURED CAUSED THE POSITIVE TERMINAL CLAMP TOARC WHICH IGNITED BATTERY GASES RESULTING INT EH TOP OF THE BATTERY EXPLODING OFF AN DSTRIKING INJURED IN THE FACE. HE SUFFERED A LACERATION OF TH ELEFT CHEEK AND SUTURES WERE REQUIRED.
THE #2 COMPACTOR MOTOR WENT TO GROUND, CAUSING THE INCOMING POWER TO TRIP OFF AT THE SUB-STATION. THIS STATION BELONGS TO SOUTHWESTERN PUBLIC SERVICE COMPANY. SPS WAS CALLED, THEY RESPONDED WITH A TECHNICIAN AND THE ELECTRICAL POWER WAS BACK ON AT 9:25 P.M.
INJURED STATES THAT WHILE DRIVING A MEMCO 102 JEEP ALONG THE POWER ENTRY OF UG AREA 287 HE STRUCK A BUMP IN THE ROAD & JAMMED HIS NECK AGAINST THE MINE ROOF CAUSING A STRAIN IN HIS NECK
EMPLOYEE FINISHED WELDING ON AN ORE CHUTE AND WAS REPLACING A PORTALBE WORK PLATFORM AT THE AMINE MIXING SCREW. HE LOOKED UP AND A DROP OF LIQUID, CONTAINING AMINE, GOT BEHIND HIS GLASSES AND INTO HIS EYES.
INJURED WAS HELPING A MAINTENANCE EE REMOVE A STRAP OFF A CONVEYOR CHAIN, WHEN A SLIVER OF STEEL CHIPPED OFF THE 4 LBS HAMMER AND STRUCK EE S RIGHT FOREARM.
IT APPEARED AS IF EE WAS CHANGING A FUSE IN THE BELT CONTROLLER ANDDID NOT HAVE THE ELECTRICAL POWER TURNED OFF, LOCKED AND TAGGED OUT.
WHILE DRIVING A DIESEL RAMCAR, INJURED STATES SHE STRAINED HE LOWER BACK WHEN THE CAR CAME INTO CONTACT WITH THE RIB AFTER A STEERING JACK HOSE BROKE.
INJURED WAS HAMMERING NAILS WHILE MAKING A BELT SPLICE, WHEN THE HAMMER(4LB) GLANCED OFF NAIL HEAD AND STRUCK HIS LEFT THUMB. HE WAS GIVEN X-RAYS AND DRESSING APPLIED BY EMERGENCY ROOM DOCTOR & PRESCRIPTION MEDICINE FOR PAIN ONLY WAS GIVEN.
INJURED STATES HE WAS REACHING FOR A ROOF BOLT WHEN HE FELT A STRAIN IN HIS BACK.
INJURED STATED HE WAS INSPECTING THE RETURN AIRWAY FOR THE UNDERGROUND AREA 250, WHEN HE ENCOUNTERED A LIGHT CLOUD OF ELECTRICAL CABLE SMOKE. THE ELECTRICAL CABLE FOR THE AREA EXHUAST FAN HAD MALFUNCTIONED AND WAS BEING SERVIECED BY THE AREA MECHANICS INBY OF INJURED. THE CABLE SMOKE CAUSED AN IRRITATION IN INJURED'S LUNGS & HE RECEIVED MEDICAL TREATMENT, PRESCRIPTION ONLY 2-27-95.

K-102 CRYSTALLIZER LOST CIRCULATION BECAUSE THE DOWN COMER HAD PLUGGED. EMPLOYEE WAS GOING TO THE TOP FLOOR TO BREAK THE VACUUM. AFTER HE GOT ON THE MANLIFT, THE DOWN COMER CAME UNPLUGGED, CAU SING THE TURBIN TO OVER SPEED & KICK OFF. WHEN THE TURBINE KICKED OFF, THIS ALLOWED THE VAPOR CHAMBER TO EMPTY INTO THE BODY OF THE CRYSTALLIZER & RUN OVER, HE WAS UNABLE TO GET OUT OF WAY OF
EMPLOYE EWAS TIGHTENIGN BOLT SIN THE RIGHT GATHERING ARM BEARING RETAINER CAP. HIS RATCHET SLIPPED OFF BOLT HEAD, CATCHIG HIS FINGER BETWEEN THE RACHET AND GATHERING HEAD PLOW.
TWO CREWS WERE TAKING THEIR TOOLS FROM LOCKERS LOCATED AT AREA 200. THE CREW ON #3 MANTRIP WERE LEAVING FIRST. WHILE BACKING UP TO TURN AROUND. THE MANTRIP BUMPED INTO P-16 SCOOTER. THE MANTRI P PULLED FORWARD FORCING THE SCOOTER INTO INJURED, PINNING INJURED BETWEEN THE LOCKERS & THE FRONT OF THE SCOOTER.
INJURED AND EMPLOYEE WERE WORKING TOGETHER, REPAIRING HTE CONVEYOR CHAIN ON #8650 BELT FEEDER. HE STRUCK A CONNECTING PIN WITH A HAMMER. A SMALL PIECE OF STEEL CAME OFF THE PIN AND STRUCK EE . SUTURES REQUIRED AFTER STEEL AND REMOVED FROM SIDE OF HIS NOSE.
EMPLOYEE WAS WALKING BESIDE THE #8319-MBC THAT WAS POSITIONED CLOSE TO THE MINE RIB. THE #8320-MBC OPERATOR BACKED HIS UNIT UP CAUSING THE #8319-MBC TO SWING TO THE RIGHT, MASHING EMPLOYEE'S L EFT LEG BETWEEN FRONT OF #8319-MBC AND THE MINE RIB.
EE WAS ATTEMPTING TO REMOVE LUMPS OF PRODUCT FROM THE SCREENING PLANT BY PASS GATE. THE GATE CLOSED WHEN LUMPS WERE REMOVED, CATCHING HIS FINGER.
INJURED AND OTHER EMPLOYEE WERE WORKING ON A YALE LIFT IN THE UNDERGROUND HYDRAULIC SHOP. ANDTHEY WERE DISCUSSING WHERE ONE OF THE WIRES SHOULD ATTACH. INJURED SAID "I'LL GO GET MAINTENANCE MANUAL." OTHER EMPLOYEE HEARD HIM HIT THE FLOOR OTHER EMPLOYEE RAN AROUND THE FORK TRUCK AND SAWINJURED ON THE FLOOR, TRYING TO BREATHE, AND HE CALLED FOR HELP.
ANOTHER EMPLOYEE WAS USING A 4LB HAMMER WHEN IT SLIPPED OUT OF HIS HAND, TRAVELED APPROXIMATELY 4 FEET, STRIKING EMPLOYEE ACROSS THE NOSE.
EE WAS CUTTING 1/4" PLYWOOD ON A 10" TABLESAW, AS HE WAS PUSHING THE PLYWOOD THROUGH, IT BECAME JAMMED BETWEEN THE BLADE AND FENCE. THIS CAUSED THE PLYWOOD TO KICK BACK AND TO THE RIGHT. HIS H AND CAME INTO CONTACT WITH THE ROTATING SAW BLADE.
UWHILE REMOVING HYDRAULIC COMPONENTS (PMPS, MOTORS AND VALVE BANKS (FROM TEH SHIPPING BOX AND PLACIGN THEM ON SHELVES IN U.G. WAREHOUSE. HE EXPERIENCED PAIN IN HIS LOWER BACK. EMPLOYEE CONTI NUED TO WROK BUT HE PAIN CONTINUED TO WORSEN.
HOIST WENT DOWN ELECTRICALLY. DURING THE IMMEDIATE INVESTIGATION THE HARMONIC FILTER WAS FOUND TO BE BAD.
UHM IN E PANEL SHORTED OUT.
WHILE HELPING WITH REPAIRS ON THE CONVEYOR CHAIN FOR 8643 BELT FEEDER. SOME FOREIGN PARTICLES POTASH ORE GOT INTO HIS LEFT EYE. HIS EYE BECAME INFECTED AND HE WAS UNABLE TO CONTINUE WORKING HI S FIRST DAY TO MISS WORK WAS 10-27-93.
THIS HOIST WAS SHUT DOWN SO ROUTINE SEVICE WORK COULD BE PREFORMED AFTER THIS WORK WAS COMPLETED, THE HOIST WOULD NOT START UP. AN IMMEDICATE INVESTIGATION SHOWED A GROUND FAULT.
THE INCOMING POWER SUPPLY FROM SOUTHWESTERN PUBLIC SERVICE COMPANY WENT OFF & THE AUTOMATIC SWITCHES DID NOT RE-SET
THE ELECTRICAL POWER WAS TURNE DOFF FOR THE HOIST, IN ORDER TO CHAGNE OUT A 4160 VOLT. SWITCH GEAR. AFTER THE SWITCH GEAR WAS CHANGED AND ELECTRIC POWER TURNE DON, THE AREA HOIST WOULD NOT START.
THE WINDER MOTOR WENT DOWN ELECTRICALLY. DURING THE IMMEDIATE INVESTIGATION THE #170 TRIGGER BOARD WAS FOUND TO BE BAD.
HE WAS IN THE PROCESS OF CHANGING THE HYDRAULIC SUMP PUMP, WHEN HE FELT PAIN IN HIS LOWER BACK. THIS PUMP IS LOCATED IN SUCH A MANNER, THAT ONLY ONE PERSON CAN WORK ON IT AT A TIME.
WHILE CHANGING THE HYDRAULIC PUMP ON 9082 SCOOP TRAM HIS ARM POPPED AT THE ELBOW. HE WAS SENT TO A DOCTOR, BECAUSE OF PAIN IN THE ARM.THIS DOCTOR DIAGNOSED THE CONDITION AS A PULLED MUSCLE. EMPL CONTINUED TO WORK, & HIS CONDITION WORSENEDON 8/24/93 HE WAS SENT TO ANOTHER DOCTOR THAT REVIEWED THE XRAYS & FOUND A FRACTURED BONE.
WHILE OPERATING UTILITY FEHICLE #9076 WITH HIS FEET OUTSIDE OF THE OPERATORS COMPARTMENT HE MASHED BOTH FEET & ANKLES BETWEEN THE SIDE OF THE VEHICLE & THE 4160 VOLT CABLE SLED THAT HE WAS MOV ING
EE WAS INSTALLING A NEW CONDUIT ALONG 3105 BELT LINE. HE STEPPED OFF THE BELT STRUCTURE TO THE GROUND. CAUSING HIS KNEE TO TWIST AND POP. RUFUS CONTINUED TO WORK BUT WAS HAVING A PROBLEM WITH THE KNEE AN MRI WAS RUN AND IT REVEALED THAT THECARTILAGE WAS TORN HIS FIRST DAY OFF WORK WAS 7-20-93.
THE WINDER MOTOR WENT DOWN ELECTRICALLY & WOULD NOT RE-SET. AN IMMEDIATE INVESTIGATION SHOWED SOME CARBON & MOISTURE BUILD UP IN THE BRUSH RINGS.
MOISTURE HAD COLLECTED IN THE WINDER MOTOR OVER THE REGULAR SCHEDULED DOWN WEEKEND BECAUSE THE AIR CONDITIONING SYSTEM HAD BEEN LEFT ON SINCE FRIDAY 6/18/93.
