Dust hose to the dust collector pot became full of mud. EE removed the hose at the lower point to to clear it. The hose fell off. employee continued to unclog the hose by elevating it the middle. EE ended up moving hose off to the side to allow it time to dry out. The next day EE returned to installed it. The employee said EE did not need medical attention till 4-22-26

Showing 50 of 54Employee was underneath an articulated haul truck setting jacks and cribbing. The job site was very muddy and while twisting and grabbing equipment they felt their left knee pop. Called nurse triage, they advised visiting urgent care. Doctor orders from urgent care stated employee needed to be off of the appendage and work until an MRI can be taken. MRI scheduled through work comp.
Employee was in the Secondary tower running the plant when employee noticed that the feed stopped due to a plug. Employee left the tower and Locked Out the #1 belt and the feeder pan in the tunnel. Employee went to inspect the plug. Employee put on gloves and attempted to remove the plug. While removing the rock, employee's finger was smashed between 2 rocks.
Employee was using a Torque Multiplier. The extension slipped out of the mechanics hand and come back around and struck EE in the left forearm. This caused a laceration in the back side of the forearm.
Employee reached out to install a ground clamp for the welder, as EE brought EE's right hand back to EE's body, EE's hand struck the damaged metal guard EE was about to repair causing a laceration to EE's right palm.
Not a miner, Commercial DOT Driver, Driver had staged truck in the front parking lot. The driver was climbing out of the truck and lost grip. Tried to catch self and separated shoulder.
Socket on an impact drill come off and hit employee in the month causing an fracture to two front teeth.
Employee was using an air impact wrench tightening a nut when EE got left ring finger caught behind the washer.
Employee was greasing D6T Dozer front right lift cylinder when employee's foot slipped off the track. The employee twisted and fell back but caught self on the blade.
Employee was in haul truck when they claimed a boulder shook the cab while being loaded. Causing a back strain.
Employee attempted to throw metal pipe into roll off bin pipe struck bin and came back and struck employee in the left ear causing laceration that required stiches.
Employee was walking on ground level pathway when the employee slipped on surface and fell to ground. Employee straightened left leg out and felt pop. Employee worked rest of shift. Was seen by doctor approximately 7:00pm that night and was diagnosed with a dislocated left patella. Put in brace and given prescription and was released to light duty.
Employee received a contusion from hammer bit that struck ee.
Employee was operating a skid steer on an small drainage berm when the machine become over balanced and turned on its side causing minor damage. Case was reported on 3-31-17 to the EHS department and the case become an reportable injury on 4-1-17.
Employee was walking from the parts trailer to the shop when knee popped and gave out.
EE had surgery to repair an elbow tendon tare on 1/14/16 for an incident that occurred in March of 2014. EE believed the injury to be minor at the time and did not seek treatment or report to EHS. EE reported the incident in July of 2015 and a WC claim was filed. Ins. Co. investigated the claim and accepted claim in Dec of 2015 scheduling surgery on 1/14/16. Expect 10-14 LTD.
On 7/15/2015 EE's right index became caught between the timber head and head cable of the ramp barge while taking the cable over the timber head. Tip of finger was surgically amputated the same day. EE was released to restricted duty following finger surgery. 7000-1 will be updated when EE is removed from restricted duty.
On February 27, 2015 welder received welding flash burn to eyes. Receive prescription strength ibuprofen, Tylenol and antibiotic eye drops. Received two days restricted duty and released to full duty on 3/2/2015.
Miner suffered broken bone in left hand while prepping conveyor for transport. Hand was caught between portable 5th wheel plate arm and conveyor frame as miner was moving the plate into transport mode. Surgery was conducted the same day, hand placed in cast and miner was released to light duty following surgery.
Employee was using a pry bar on a lodged rock in a pulley when it come loose. The employee fell back on his left leg causing injury to the ankle.
Employee has stated his back injury is due to a faulty seat while operating a 400D John Deere haul truck.
While working on the night shift, the employee was walking from the control tower to the metal detector, which needed to be reset, the employee slipped on the ground and injured his right knee. Incident originally occurred on 12/6/13, then became recordable on 3/25/14 after orthopedic consultation.
The belly pan of a Cat 730 haul truck fell on an employee's wrist casuing a break to his right wrist.
Employee was repairing an axle trunnion and while setting it on the ground, left hand middle finger became trapped between the part and a bolt causing fracture and laceration to finger.
Employee received a flash burn to his eyes while working in close proximity to welding operations.
Crew was working on screen unit. EE was working on west side at the discharge end of the screen unit. He was reaching to tighten a bolt on the mahcine, when he was finished he stepped back and fell off the end of the cat walk and fell to the ground 6 feet. He landed hitting his head on rocks on the ground.
EE was cutting on a seam on a convey belt making repairs. He was cutting the belt with a box knife. He was cutting the belt towards him when the knife slipped off the belt hitting him in the right knee causing a small laceration to the knee.
The hole cover had become dislodged and came free of the platform during transport.The employee put the cover back up on the platform and stated that it's not in the right place then he accessed the platform to unhook a chain from the loader.The employee then inadvertently stepped on the cover and since the cover was not in its proper place it allowed him to fall through the hole.
Employee was shoveling around the conveyor feed box to clean up the area & placing material on the #1 belt when dirt entered his eye.
Employee was using a pry bar to clear a blockage in the crusher which was locked and tagged out. After using the bar for about 15 minuted he felt a pain in his right shoulder. Medical treatment began 10/16/08 but this incident did not become recordable until 12/2/08.
Replacing impact belt. Machine was locked out and tagged out. Employee pulling on a wrench and his hand got caught between the wrench and guard pinching his hand.
Picked up a cribbing block and put it in the back of a truck. The block rolled back smashing his finger between the block and the side of the truck.
Unk at this time: possible aneurysm/stroke due to diabetes. Not beleived to be related to work. EE began feeling poorly while operating frt end loader. Transported to hospital, died at hospital possiblly from multiple strokes. We have requested & are waiting for death certificate. Med info & reportability are unk.
cutting a piece of new side belting when knife slipped and cut right knee
While doing maintanence on a locked out, tagged out not running vibrating feeder pan, employee inadvertently rotated a pulley and pinched right thumb.
Employee claims he was moving a fuel hose when he aggravated his left shoulder. This employee had previously injured this shoulder in a NON-WORK related accident. This was not recordable until he went to the doctor on May 4, 2007 and received a cortisone shot in his shoulder.
Employee was cleaning rock out of a plugged chute on screen deck when she suffered a left shoulder strain.
Ee was tarping a load and the tarp fell into the back of the truck. He went into the truck bed to retreive the tarp and as he picked it up he noticed a catch in his shoulder. He refused medical treatment until 9/15/06, and at this point it became MSHA reportable.
Employee alleges that his back was injured by using a skid steer loader. Alleged injury has been under investigation by insurance carrier ACE USA and attorney, until 10/14/06 when it was determined to be work related.
EE INJURED BACK & SHOULDER WHEN HE FELL FROM LADDER TO GROUND.
EE OPENED SIGHT DOOR OF OIL BURNING STOVE TO LOOK IN AND SOOT BLEW INTO HIS EYES CAUSING ABRASIONS.
EMPLOYEE STAYED IN THE CONTROL TRAILER WHEN A SHOT WAS PUT OFF. ROCK STRUCK AND PENETRATED THE TRAILER CAUSING A LACERATION TO EMPLOYEE'S HEAD (REQUIRING STITCHES) AND STRUCK EMPLOYEES LEFT LO WER LEG CAUSING A HAIRLINE FRACTURE AND A KNEE STRAIN (REQUIRING) PHYSICAL THERAPY.
EE DID NOT HAVE AN ACCIDENT & DOES NOT KNOW CAUSATION. ELBOW HURTS AFTER RUNNING DRILL FOR 30 MINUTES.

EE SLIPPED WHILE COMING DOWN LADDER OF CRANE ANDRECEIVED SMALL LACERATION(STITCH) TO UPPER THIGHAREA OF LT. LEG.
EE WELDED A PLATE TO THE FLOOR LINER OF THE TELESMITH TO SUPPORT A HYDRAULIC JACK. SUPPORT CAME LOOSE STRIKING EE IN RIGHT SHIN.
EE WAS WELDING AND MAINTAINING EQUIPMENT IN 100 WEATHER. EE BECAME DEHYDRATED FROM WORKING IN SUCH CONDITIONS.
PULLED SHOULDER WHILE CLIMBING DOWN LADDER.
EE WAS POUNDING ON SOUTH CRUSHER AT SECONDARY. THE HEAD OF THE SLEDGEHAMMER MISSED THE OBJECT AND THE HANDLE HIT THE WEDGE CAUSING IT TO PINCH HIS FINGER. LACERATION (STITCHES) TO LEFT INDEX F INGER.
