Struck by rolling or sliding object

Emp was stuck by a angle iron brace that was being used to hold up a roll of belting. The angle moved and the piece struck them in the left shoulder causing them to fall to the ground. They worked the rest of the shift and went to the ER after the shift. They treated them for left shoulder and elbow pain. Employee was placed off work until next Tuesday, 11/18/25.

Miner picked up can of break cleaner out of service truck walking to back of truck and slipped on some rock causing miner to start falling. Miner reached out with right hand to stop fall causing break cleaner can to hit rear bumper of truck. The can exploded in miners hand causing lacerations to EE's index and middle fingers.

Employee was sticking barge for draft and stern wire broke hitting employee in the lower right leg. Employee's lower right leg and ankle were broken as a result. Employee was given first aid and transported by ambulance to Hospital.

Struck by falling object

Employee was working on conveyor pully when it slipped striking employee in Right neck and shoulder. Employee reported to supervisor but did not want to go to doctor. Pain never went away and told supervisor EE wanted to go to doctor to be checked out. Employee was taken to doctor on 08/02/2022. Employee was given prescriptions and put on light duty.

Employee was pulling a chain out from under a parts box and tore left bicep.

Fall onto or against objects

Employee was helping in hooking up a plastic 8"" pipe to pump. Pipe was being pulled with strap and equipment to pump. The pipe broke friction from windrow and struck employee pushing employee into stationary pump. This caused a contusion on employee's back.

Over-exertion in pulling or pushing objects

Employee was pulling barge rope and strained a shoulder. Employee didn't feel like they needed to go to doctor. On Sunday June 7th employee called supervisor and said they needed to go to doctor because shoulder was hurting worse. Supervisor sent employee to ER because they were the only place open on Sunday, ER released employee to light duty.

Over-exertion in lifting objects

Miner was lifting 2 sample buckets into the back of pickup truck and felt a pull in EE's back. Employee strained EE's back. Was taken to clinic and Doctor put employee on light duty for strained back.

On 7/15/2015 EE's right index became caught between the timber head and head cable of the ramp barge while taking the cable over the timber head. Tip of finger was surgically amputated the same day. EE was released to restricted duty following finger surgery. 7000-1 will be updated when EE is removed from restricted duty.

Flash burns (welding)

On February 27, 2015 welder received welding flash burn to eyes. Receive prescription strength ibuprofen, Tylenol and antibiotic eye drops. Received two days restricted duty and released to full duty on 3/2/2015.

Clevis holding cable broke and struck EE on left leg. At the time of accident it was a first aid DR visit only all x-rays were negative. Two follow up DR visits because of light pain, Dr ordered MRI and referred to Orthopedic. Results torn ACL and surgery was scheduled to repair on 11/20/14. Employee had outpatient surgery and was released to LD the same day.

Fall from ladders

Employee was descending a ladder into a hopper barge when the ladder slipped on the barge floor. He lost balance and fell approximately 12' to the bottom of the barge fracturing his left ankle, fracturing his left elbow and compressing the L2 disk in his back.

Employee tripped and fell, did not want to go to dr. Feb decided to seek medical attention, medical provider x-ray negative, set up MRI, results read March 25, out patient surgery on March 28 released to restricted duty March 29.

Employee was working on Snubber Pully on conveyor. When last bolt was removed the pulley rolled foward and pinched right index finger between impact wrench and angle iron resulting in a cut and broken right index finger.

Employee was working on a conveyor belt; wrench slipped causing him to strike his hand against the conveyor resulting in a broken little right finger.

Employees lifting I-Beam to place on lowboy. Beam bumped track of trackhoe and pushed end around knocking employee on ground against berm and beam caught foot. Steel toed shoes, hard hat in place. No damage to mine equipment.
