Showing all 41Employee was doing repair work to oversize chute at the plant when ee went to lift a 10x2 piece of channel iron and it slipped from right hand and fell on left hand smashing left ring finger.
Employee was swinging the arm of a hoist over, to prevent a chair (in which they were going to hoist up to button house) from catching on the pea gravel conveyor, and while pulling on the bottom of chair it came loose and employee lost EE's balance and fell on EE's left elbow.
Employee was using a sledge hammer to remove a pin from track hoe bucket. While striking the pin, a piece of the metal pin sheared off into employee's right shoulder.
EE (button house operator) on ground cleaning conveyor by spraying water onto belt, noticed a clot had stopped belt from moving, EE begin to run toward button house to shut power off to keep from damaging conveyors. Shortly after EE begin to run EE turned to observe the belt and when turning back made contact with a 3" iron angle support in the nose and mouth area.
Employee was using a sledge hammer to drive a pin out from a 330 CL track hoe. Had pain in right shoulder. Note: Employee previously had a similar claim on June 26, 2015, esulting with same complaint, (pain in right shoulder.)
Employee was walking the conveyor system at a incline and began to fall and threw his right hand out to catch himself and fell on hand. (Per phone call he was working outside and fell to the ground. Hand was strained.)
Employee was sitting in chair and he turned in chair to stand up and hit the bottom of chair with right foot. He heard his right hip pop and begin feeling pain in right thigh and hip.
Employee shoveling material from under conveyor belt and lost balance and fell. Hurt right knee.
Employee was rolling a huge clod of material down the stockpile, when he heard a pop from his right arm and he began to feel pain. He also had swelling in upper arm. He said the pain was tolerable the last few weeks but arm is really no better.
Employee was replacing a hydraulic hose on a 966F loader. He was using a combination wrench to tighten the hose while standing in a awkward position, he felt pain in his right shoulder. Later he grasped the ladder on a 980G loader, that he was repairing and felt the pain again.
Hit a pin with a 20 lb. sledge hammer on a 980 g loader boom frame and upon contact a sharp pain and popping noise was experienced momentarily in left shoulder.
Cleaning return roller with shovel and return roller pulled shovel and right arm between conveyor belt and return roller bruising the right arm and wrist.

Mechanic was working in shop installing a starter through a hole in the equipment, while doing so Employee strained his left shoulder.
Employee laid tools on screen up on vibrator and when screen moved, the tools started falling and employee reached to grab them, causing him to fall.
Without the control tower knowing, EE sees belt not running, removes guard off belt drive and puts his hand into it. He failed to realize his hand was between belt and pulley.

Employee was shoveling from under crusher when an oversize rock (2 to 3 inch rock) fell from crusher and hit employee on the right temple.

EMPLOYEE WAS WORKING ON SCRAPPER MACHINE AND A PART OF THE MACHINE HE HAD TAKEN OFF (BRACE) AND LAID AGAINST THE MACHINE TIPPED OVER HITTING THE EMPLOYEE BEHIND THE RIGHT KNEE (CALF MUSCLE).
EE HAD GOTTEN OUT OF TRUCK TO CHECK WATER DISCHARGE, ABOUT 20' FROM TRUCK EE SLIPPED IN THE MUD AND FELL TO THE GROUND.
EMPLOYEE WAS SHOVELING UNDER CONVEYOR NEAR RETURN ROLLER. THE ROLLER CAUGHT THE SHOVEL AND PULLED EMPLOYEE'S ARM UP TO THE ROLLER, CAUSING A MUSCLE STRAIN.
ADJUSTING TAIL PULLEY ON SAND CONVEYOR, SLIPPED INTO TAIL PULLEY INJURING LEFT THUMB & LT. RING FINGER.
EE WAS HELPING TO LOAD CUTTING EDGES AND SUNK INMUD. HE SLIPPED AND TWISTED HIS KNEE, HITTING ITON THE CUTTING EDGE.
EE WAS WORKING ON A 627 SCRAPER, REPAIRING CUSHION HITCH, WHILE TRYING TO PUT A WOOD BLOCK UNDER CUSHION HITCH, CUSHION HITCH FLOATED DOWN AND CAUGHT HIS FINGER BETWEEN WOOD AND HITCH.
EE SHUT HIS HAND UP IN HIS VEHICLE DOOR.
EE WAS USING A CUTTING TORCH CUTTING SCREENS WHEN A PIECE OF METAL FLEW INTO HIS RIGHT EYE SCRATCHING HIS CORONA.
