Jonesboro Plant 675Metal/Nonmetal

Employee was sitting in chair and he turned in chair to stand up and hit the bottom of chair with right foot. He heard his right hip pop and begin feeling pain in right thigh and hip.

Employee washing out feeder belt, went to put hose and walked back out and slipped/jarred himself trying not to fall, straining his right leg & groin area.

Employee was climbing down from side of the chute and slipped and fell approximately 5 ft and hit a guard with his left side, resulting in three cracked ribs.

Steering lock to left and brake pedal went to the floor and dump bed overturned.

ADJUSTING TAIL PULLEY ON SAND CONVEYOR, SLIPPED INTO TAIL PULLEY INJURING LEFT THUMB & LT. RING FINGER.
