Struck against stationary object

Employee was working as ground man and was walking near the crusher, and stepped on a rock and injured right foot.

EE (button house operator) on ground cleaning conveyor by spraying water onto belt, noticed a clot had stopped belt from moving, EE begin to run toward button house to shut power off to keep from damaging conveyors. Shortly after EE begin to run EE turned to observe the belt and when turning back made contact with a 3"" iron angle support in the nose and mouth area.

Over-exertion in wielding or throwing objects

Employee was using a sledge hammer to drive a pin out from a 330 CL track hoe. Had pain in right shoulder. Note: Employee previously had a similar claim on June 26, 2015, esulting with same complaint, (pain in right shoulder.)

Fall to the walkway or working surface

Employee shoveling material from under conveyor belt and lost balance and fell. Hurt right knee.

Over-exertion in pulling or pushing objects

Employee was replacing a hydraulic hose on a 966F loader. He was using a combination wrench to tighten the hose while standing in a awkward position, he felt pain in his right shoulder. Later he grasped the ladder on a 980G loader, that he was repairing and felt the pain again.

Over-exertion in wielding or throwing objects

Hit a pin with a 20 lb. sledge hammer on a 980 g loader boom frame and upon contact a sharp pain and popping noise was experienced momentarily in left shoulder.

EMPLOYEE WAS MOUNTING MOTOR, MOTOR SLIPPED AND CAUGHT EMPLOYEES LEG ON THE KNEE AREA, CAUSING DISLOCATING KNEE AND BRUSING.

Without the control tower knowing, EE sees belt not running, removes guard off belt drive and puts his hand into it. He failed to realize his hand was between belt and pulley.
