An employee was using a 3" high pressure water hose to fill an equipment pressure washer and the employee lost control of the water hose and it slapped the employee in the left shoulder and chest, causing a strain to the upper left arm tendon, shoulder muscle and chest.

Showing all 23Scale Master, slipped and fell, walking down aggregate and dirt embankment. They fractured the right side of their left ankle and caused a hematoma on the left side of their left knee.
Fell on right shoulder, bruised.
3 inch rock bounced off shaker screen and hit operator in the mid back causing a small bruise.
Employee fell 4'to 5'off a ladder while working on a portable crusher. Bruising their right elbow and right hip.
The crusher operator was climbing down the ladder on the crusher and missed a step and fell 4 1/2' to the ground, fracturing the bone in lower left leg.

Miner tripped on his own shoe string and fell. Dislocated right thumb. Took him to the Clifton Medical Clinic and they relocated thumb and put in stitches.
Based on investigation it is belived that EE climbed up on top of the pan feeder on an extec crusher while it was in operation. This was in violation of company policy & training. EE went through the crusher and was killed.
Employee was cleaning tailpulley with prybar and belt was turned on. Note: Employee quit going to the doctor.
Driver was getting out of truck, foot slipped and was falling and put right hand on ground to catch himself.
